Drawing on various disciplinary perspectives, this book re-focuses the debate on what makes a good health system, with a view to clarifying the uses of social science research in thinking about health care issues in Africa. The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

Ce travail d'étude et de recherche a pour objet l'éducation du patient chronique autrement dit, diabétique, asthmatique, alcoolique, etc. Cette démarche éducative s'intègre aujourd'hui, de façon plus sytématique et structurée, dans la pratique thérapeutique. Elle tend à dévenir un élément central des soins. Aussi est-il question dans ce mémoire de comprendre comment les professionnels de santé de milieu hospitalier (médecins et infirmières) conçoivent et intègrent l'éducation du patient dans leur démarche de soins. L'enjeu est ici de comprendre le sens donné à cette pratique, de voir son état de développement, les modifications provoquées concernant la relation soignant-soigné, l'identité de thérapeute ou encore l'organisation du travail, et de repérer d'éventuels changement socialement souhaitables (ou souhaités). La méthode utilisée dans cette recherche fut des entretiens et quelques observations de séances éducatives de groupes. Les résultats montrent que les soignants n'ont pas tous la même approche théorique et pratique de l'éducation du patient. Des débats sont encore présents sur le statut de cette dernière et concernant le rôle de "soignant-éducateur". En outre, cette démarche est encore difficilement intégrée et structurée dans les pratiques de soins. Des problèmes de travail d'équipe, d'homogénéisation des pratiques, de reconnaissance par l'institution hospitalière et de manques de formation à la pédagogie perdurent.

From the viewpoint of a health economist, the intensive care unit (leU) is a particularly fascinating phenomenon. It is the epitome of "high-tech" medicine and frequently portrayed as the place where life-saving miracles are routinely wrought. But the popular imagina tion is also caught up in the darker side, when agonizing decisions have to be made to avoid futile and inhuman continuation of expen sive treatments. My analytical interests led me to approach these issues by asking what the evidence tells us about which leu activities are very bene ficial in relationship to their costs and which are not. This quickly translates into a slightly different question, namely, which patients are most appropriately treated in an leu and which not. Unfor tunately, it is very hard to answer these questions because it has pro ved very difficult to investigate these issues in the manner which is now regarded as the "gold standard:' namely by conducting rando mized clinical trials or alternative courses of action. I think this is a pity, and I am not at all convinced that it would be unethical to do so in many cases, because there is wide variation in practice and ge nuine doubt as to which practices are best -the two conditions that need to be fulfilled before such a trial is justifiable.
