L'incidence de la maladie thromboembolique veineuse (MTEV) augmente avec l'âge. Sa morbi-mortalité sur les personnes âgées soulève plusieurs problématiques spécifiques de prise en charge. L'objectif de notre étude était de décrire les patients de 80 ans ou plus ayant présenté un évènement thromboembolique et pris en charge au CH Lyon Sud entre janvier 2016 et décembre 2017. Les patients inclus ont été identifiés par le Programme de Médicalisation des Systèmes d'Information grâces aux codes CIM10 de l'embolie pulmonaire (EP) et de la thrombose veineuse profonde (TVP). Les critères analysés étaient les facteurs de risque de MTEV, les méthodes diagnostiques, les modalités de traitement et le devenir à 3 et 6 mois. Les informations ont été recueillies de façon rétrospective dans les dossiers informatiques et analysées grâce au logiciel Microsoft Excel. 181 patients ont été inclus, parmi lesquels 134 avaient une EP et 97 une TVP. 140 patients (77%) présentaient des symptômes évocateurs d'une MTEV ; la dyspnée demeurait le signe le plus fréquent (48%) en cas d'EP. 41 EP (23%) ont été découvertes de façon fortuite au décours d'un examen réalisé pour un autre motif. Chez 141 patients (82 %), au moins un facteur favorisant était retrouvé, le plus fréquent étant l'alitement ou confinement au fauteuil (47%). Après la découverte de leur MTEV, 98% des patients étaient hospitalisés et 90 % ont été traités par un anticoagulant, pour une durée moyenne de 6 mois. Les anticoagulants oraux directs (ADO) étaient majoritairement utilisés (38%). A trois mois, 47 patients (26 %) étaient décédés. L'amélioration du diagnostic et du pronostic de la MTEV chez le sujet de plus de 80 ans nécessite de mieux connaitre ses particularités sémiologiques. Une meilleure utilisation des scores pronostiques et des traitement oraux directs pourraient permettre une meilleure prise en charge sans recours systématique à une hospitalisation.
