Les Troubles Musculo-Squelettiques (TMS) d'origine professionnelle sont en constante augmentation depuis plusieurs décennies. Ils représentent actuellement la majeure partie des Maladies Professionnelles Indemnisables (MPI) par le régime général de sécurité sociale. Pourtant, le suivi systématique des TMS reconnus en MPI sous-évalue l'impact de ces affections au sein de la population active, du fait de deux phénomènes : la sous-déclaration et la sous-reconnaissance des Maladies d'origine Professionnelles. L'objectif de ce travail est d'évaluer l'importance de ces deux phénomènes sur la réparation des TMS d'origine professionnelle en Franche-Comté. La méthode employée consiste à comparer, à l'échelle régionale, les données concernant les TMS du membre supérieur et du rachis, reconnus en MPI, aux données issues d'un système de surveillance épidémiologique des Maladies à Caractère Professionnel (MCP), baptisé programme MCP. A l'issue de ce travail, la sous-déclaration des maladies d'origine professionnelle apparaît effectivement très marquée, même pour des affections pouvant prétendre à une réparation au titre d'un tableau de Maladies Professionnelles. La sous-reconnaissance semble, quant à elle, se limiter essentiellement aux pathologies des épaules et du dos.
