Cette monographie est un recueil des données actualisées sur le sujet. Il est construit comme le parcours initiatique et pratique de celui qui doit résoudre le problème d'une révision de prothèse de la décision de reprise jusqu'à sa mise en oeuvre. Chaque chapitre après une présentation de son contenu et de son orientation a recours à la mise en balance ou en discussion de chacun des acquis dans le déroulement de l'enquète qui aboutit à la décision puis à la réalisation de la mise en place d'une nouvelle prothèse. Cette démarche est fondamentale quand on connaît les difficultés et par là mème les complications que génère cette chirurgie. Après avoir rappelé l'épidémiologie des reprises et la mouvance de celle-ci à travers leur courte histoire une approche soigneuse des pièges de la démarche diagnostique est présentée. Si la décision de reprise se confirme la notion de bénéfice/risque doit conduire au consentement éclairé du patient validé par l'anesthésiologiste qui doit prendre en compte les comorbités et la fragilité de ces patients souvent âgés. Dès lors l'état des lieux tant au niveau osseux que du matériel en place doit ètre effectué. Les données actuelles sont largement présentées: classifications des pertes de substances et leur évolution analyse de l'état de la prothèse initiale et de ses difficultés d'exérèse. Cette étape conduit à appréhender les pièges à éviter lors de cette chirurgie difficile. Une large place est donnée à laprésentation des techniques de prise en charge tant pour la reprise des lésions du cotyle que de celles du fémur. De multiples expériences sont rapportées et décrites avec précision par des experts. Le temps chirurgical vient ensuite exposant pas à pas les problèmes à prévenir ou à résoudre simples ou compliqués. Les orientations sont plus que de principe liées à des attitudes d'école laissant le lecteur libre de ses choix mais en le guidant au travers d'une cohérence d'action. Une synthèse pratique clôt ce chapitre. Les complications per- et postopératoires sont ensuite largement évoquées pour maintenir notre vigilance. Pour mettre l'accent sur l'importance du service rendu par cette chirurgie fonctionnelle la dernière partie s'intéresse à l'analyse de la qualité de vie au travers de scores aujourd'hui plus performants qui permettent au patient et non au seul chirurgien d'évaluer le bénéfice qu'il a retiré de ce type d'intervention. En résumé il s'agit d'un volume complet très documenté construit sur la démarche pratique de la révision des prothèses totales de hanche.

In Germanspeaking countries, Kaiser pub lished the last comprehensive textbook on congenital hip dislocation in 1958, and Schlegel followed with a comprehensive handbook in 1961. In the Angloamerican world, Coleman's monograph was pub lished in 1978, Somerville's in 1982, and Wilkinson's in 1985. Progress in medical science, and the deeping of physician experience in general, make continuing education a fundamental obligation on the part of the practicing physician. Besides academies of advanced medical training, we feel that medical schools and uni versities offer a particularly good setting for continuing medical education. When the Hannover Medical School established West Germany's fIrst Department of Trauma Surgery in 1970, we committed ourselves to this goal and instituted the Hannover Trauma Seminars as a regional forum for continuing physician education.
