Objectif : Evaluer l'effet d'une formation clinique en pédiatrie sur l'approche des étudiants de médecine générale. Méthodes : Une étude de cohorte a été menée dans notre centre hospitalier universitaire "Notre Dame des secours". Tous les étudiants de 6ème année (27) ont reçu une formation clinique portant sur l'approche des patients en pédiatrie et l'examen physique pédiatrique. Les étudiants ont rempli un questionnaire dirigé sur les différentes aptitudes d'examen physique pédiatrique. Le même questionnaire a été rempli par les mêmes étudiants en médecine à la fin de leur rotation en pédiatrie. Le groupe contrôle étant un autre groupe d'étudiants n'ayant pas bénéficié d'une formation supplémentaire avant le stage (29) et qui ont rempli le même questionnaire en fin de leur rotation en pédiatrie. Résultats : En comparant les deux groupes, 19,6% et 39,3% des étudiants en 6ème année de médecine du groupe d'intervention ont ressenti que "si un enfant pleure, c'est de leur faute" (p = 0.023) et que "la mère de l'enfant a toujours raison jusqu'à preuve du contraire" (p =0.000) respectivement vs 7,1% et 14,3% dans le groupe des contrôles. En ce qui concerne le confort de l'enfant, le groupe d'intervention a appris que "si l'enfant est chatouilleux, ses propres mains peuvent être utilisées pour palper son abdomen" (p = 0.000). Les étudiants du groupe d'intervention ont appris que "l'utilisation des robes hospitalières" et "l'explication de toutes les étapes de l'examen physique à l'enfant" peut être un bon premier contact (p=0007). En outre, "laisser l'enfant en sous-vêtements" ou "demander aux parents de quitter la salle" (p=0,025) peut optimiser l'intimité. Les étudiants peuvent aussi observer "leur marche et leur état musculosquelettique" en cas de non-coopération (p=0.030), et enfin "laisser l'enfant jouer avec les outils médicaux" peut aider à gagner leur confiance (p=0,003). Conclusion : Nous concluons que notre formation clinique supplémentaire a amélioré certains aspects de l'examen physique pédiatrique. Une étude à une plus grande échelle devrait être menée pour mieux évaluer l'avantage de l'application d'un tel changement dans le cursus de l'internat.
