Résumé : les leucémies, qui sont des hémopathies malignes, représentent le cancer pédiatrique le plus fréquent, avec 450 à 500 nouveaux cas par an en France. On rencontre principalement des leucémies aiguës qui sont caractérisées par l'expansion clonale de précurseurs hématopoïétiques bloqués à un stade précoce de leur maturation. Dans une première partie, nous allons décrire la physiopathologie, le tableau clinique, les signes biologiques, la classification des leucémies aiguës, ainsi que les différents protocoles thérapeutiques actuels qui ont permis d'augmenter de façon significative la survie. Dans un second temps, nous mettrons en évidence le rôle primordial du Chirurgien-Dentiste dans la prise en charge des enfants atteints de leucémie aigüe, à tous les stades de la maladie et en collaboration avec l'équipe médicale. Il peut tout d'abord jouer un rôle dans le diagnostic de la maladie, certaines manifestations buccales pouvant être un signe d'appel d'une leucémie sous-jacente. Ensuite, un enfant atteint de leucémie aiguë doit bénéficier d'un bilan bucco-dentaire complet avant la mise en place du traitement oncologique afin de limiter les complications. Une prise en charge spécifique est indispensable au cours du traitement par chimiothérapie, radiothérapie ou par greffe de cellules souches hématopoïétiques. En effet certains traitements entrainent une toxicité directe (ex : mucites) et indirecte (ex : infection opportuniste due à l'aplasie médullaire). Enfin, le rôle du Chirurgien-Dentiste est prépondérant dans le suivi des enfants en rémission car les différents traitements utilisés pour la leucémie aiguë sont associés au développement possible de complications bucco-dentaires à long terme (agénésies, anomalies de structure, microdontie, anomalies radiculaires...) et qui nécessitent un suivi dentaire régulier.
