Le diabète sucré est une pathologie chronique aux graves complications. Le nombre croissant de malades à travers le monde et le coût élevé de leur prise en charge font du diabète un problème majeur de santé publique. En France, les décideurs ont évalué les difficultés diagnostiques et thérapeutiques. Ils tentent maintenant de réorganiser les systèmes de soins concernant le diabète et globalement les maladies chroniques. A La Réunion, le diabète est actuellement une priorité régionale de santé à cause de sa forte prévalence et d'une situation de soins insatisfaisante. Celle-ci s'expliquerait principalement, selon nous, par l'absence de prise en compte du particularisme individuel dans la prévention ou le traitement du diabète, outre le rôle de l'histoire médicale du diabète, creuset commun des mentalités des praticiens et des profanes, ou celui des caractéristiques pathologiques et des conceptions populaires issus du XVIIème siècle. La recherche anthropologique porte sur une quarantaine de diabétiques de type 2, réunionnais, interrogés surtout en secteur hospitalier. L'analyse de leur discours permet de saisir le sens de leur itinéraire de soins, déterminé par leurs conceptions étiologiques et leurs choix thérapeutiques. L'approche de l'hérédité, de l'alimentation et des croyances traditionnelles précise les représentations biomédicales et profanes du diabète. L'analyse des comportements face aux facteurs de risque, comme l'obésité, montre l'écart entre les valeurs des soignants et celles du public, en particulier féminin. Les femmes ont un rôle social prépondérant en tant que médiatrices des messages sanitaires, non exploité par les soignants. Les actions des professionnels révèlent des positions figées et d'autres négociées. On en conclut qu'il est capital de mieux connaître "l'espace anthropomédical" du malade pour l'aider à continuer à "vivre avec"sa maladie et à gérer la maladie chronique de façon autonome.

Indigenous Peoples and Diabetes is a bold attempt to reframe the meaning of diabetes mellitus as a socio-political disorder from the perspective of Indigenous Peoples, community workers, medical anthropologists, and health professionals working and/or living in North America, Latin America, the Arctic, Australia, and the Indian Ocean. The anthology discusses the effects of social history on the etiology and epidemiology of type 2 diabetes within Indigenous experiences of cultural expansionism and colonial occupation. Indigenous narratives about the right to food, health, emotional experience, and the importance of networks of solidarity provide reflective critiques on community wellness, empowering individuals to regain control of their health, spiritual knowledge, and emotional liberty. The book is a paradigm-breaking endeavor because it challenges the widespread assumption that Indigenous Peoples all over the planet are inherently susceptible to sicken and die from degenerative ailments such as diabetes because of their faulty genotype, poor dietary habits, and sedentary lifestyle. Instead, the creative assemblage of chapters shifts the medical gaze from a potentially diseased body to a diseased colonial and post-colonial history of genocide practiced against Indigenous Peoples to this day. Innovative programs to combat the diabetes epidemic and promote physical and emotional wellness are discussed in detail, such as the Mino-Miijim 'Good Food for the Future' program on the White Earth Reservation in Minnesota; the Kahnawake School Diabetes Prevention Project developed in the Kanien'keha':ka (Mohawk) community of Kahnawake, near Montreal, Canada; and the Cultural Rebuilding Project at the Potawot Health Village in northern California. The authors are inspired by a strong commitment to a liberation medicine and to the belief that access to good food, respect for cultural traditions, and integrative therapies are basic human rights. Le terme promotion de la santé en Afrique, près de 30 ans après l'adoption de la Charte d'Ottawa, continue d'avoir des connotations complètement hors du sens que lui confère cette charte. Cela n'est pas étonnant quand on sait que la notion de santé dans ce contexte africain équivaut à la lutte contre la maladie à travers les soins de santé dispensés par des professionnels de la santé dans des formations sanitaires et les hôpitaux. L'évolution que connait le continent depuis quelques décennies est de donner un peu plus de place à la communauté à travers les relais communautaires dans une participation communautaire vidée de son contenu, car le pouvoir n'est jamais passé entre les mains des communautés.C'est au vu de tout ceci que le présent ouvrage à sa raison d'être pour expliquer les fondements de l'autonomisation communautaire et de la promotion de la santé avec leur importance pour la région africaine en proie aux mauvais indicateurs de santé comparativement aux autres régions du monde.
