Introduction. Le cancer du sein triple négatif (CSTN) se distingue par une population plus jeune avec un pronostic plus sombre. L'objectif principal de cette étude est d'apporter une réflexion supplémentaire pour appuyer la nécessité d'un acteur central qui serait l'Onco-Sénologue pour une prise en charge optimale et coordonnée du CSTN. Matériel et Méthode. Il s'agit d'une étude rétrospective et bicentrique portant sur les patientes suivies pour un CSTN dans deux centres de soins en Lorraine. Ont été incluses toutes les patientes suivies entre 2010 et 2015 pour un CSTN. Plusieurs paramètres ont été étudiés et particulièrement la consultation onco-génétique, la consultation de fertilité, la décision d'une chimiothérapie néo-adjuvante ou adjuvante, et l'envahissement ganglionnaire. Résultats. Sur plus de 3000 patientes atteintes d'un cancer du sein suivies sur la dite période, 340 avaient un CSTN.L'âge médian était de 54 ans et77% (n=263) des tumeurs étaient de grade 3. On pouvait noter 16% (n=54) et 36% (n=13) de cas de discordances entre les recommandations et les pratiques concernant respectivement la consultation oncogénétique et de fertilité. Il y avait 67 cas de chimiothérapie néo-adjuvante et 273 cas de chimiothérapie adjuvante dont les principaux facteurs décisionnels étaient l'âge, la taille tumorale et la présence d'emboles. Conclusion. Afin d'optimiser la prise en charge du CSTN en particulier et par extrapolation celle du cancer du sein en général, il serait pertinent de centrer celle-ci par un acteur dédié dont la mission serait de coordonner l'ensemble de ces éléments et qui serait l'Onco-Sénologue.
