Contexte. Le cancer du sein chez la femme jeune nécessite une prise en charge spécifique, au vu de ses enjeux sur la fertilité, la génétique et les problèmes psycho-sociaux. Le cancer du sein métastatique (CSM) de novo est peu fréquent avec peu de données disponibles. Méthode. Nous avons évalué la prise en charge et le pronostic des femmes de = 40 ans suivies pour un CSM de novo dans la cohorte nationale multicentrique du programme ESME, regroupant 22 463 patientes traitées pour un cancer du sein entre 2008 et 2016. Toutes les patientes ayant un statut RH (récepteurs hormonaux) et HER2 connu ont été incluses. L'objectif principal était de comparer la survie globale (SG) des femmes de = 40 ans à celles des patientes plus âgées, globalement et dans chaque sous-type tumoral. Résultats. Sur les 5 997 femmes identifiées avec un CSM de novo dans la base de données, 598 (10%) avaient = 40 ans. L'âge médian au diagnostic était de 36 ans. La majorité avait une tumeur RH+/HER2- (n=289; 48.3%); tandis que 207 patientes (34.6%) avaient une tumeur HER2+ et 102 (17.1%) une tumeur HR-/HER2- (triple négative, TN). En comparaison avec les femmes de > 40 ans, les femmes de = 40 ans avaient plus fréquemment des tumeurs de grade III (49% vs 33.8%), HER2+ (34.6% vs 24.3%) et TN (17.1% vs 12.1%), p
