La mort foetale in utero est une situation qui s’immisce en maternité et frappe les parents comme les sages-femmes qui sont alors en première ligne. Ce mémoire se propose d’identifier les principales difficultés que les sages-femmes rencontrent lors ces prises en charge. Une étude basée sur des entretiens semi-dirigés avec des sages-femmes, de maternité de type un et trois, ayant déjà pris en charge des morts foetales, in utero nous a permis de nous éclairer quant aux difficultés rencontrées. L’absence d’anticipation, le retentissement émotionnel sur les sages-femmes et le décalage entre l’approche théorique et la confrontation avec la mort ont été les principaux thèmes abordés. Il est nécessaire que les sages-femmes identifient leurs difficultés afin de les surmonter et permettre alors une meilleure prise en charge, et qu’une approche soit plus concrète lors des études de sages-femmes.

METHODE - Une étude observationnelle descriptive a été menée au sein de la maternité Olympe de Gouges du CHRU de Tours du 1er novembre 2018 au 7 janvier 2019. Des questionnaires imprimés ont été distribués aux sages-femmes de salle de naissance, de suites de couches physiologiques et pathologiques, d'unité de grossesses pathologiques et de consultations prénatales. RESULTATS- La majorité des sages-femmes de notre étude ont Lm ressenti positif lors de l'accompagnement de patientes présentant une MFIU malgré des difficultés émotionnelles et administratives. Les outils mis à leur disposition sont, en grande partie, suffisants pour gérer les difficultés énoncées. CONCLUSION- Une grande empathie est ressentie au cours de cet accompagnement ayant des bienfaits pour les patients et les saignants. Celle-ci peut être à l'origine d'un sentiment d'utilité pour le couple. La m01i ne laisse pas indifférent et, comme la plupart des saignants, les SF ne s'y habituent jamais. L'accompagnement est unique et présente ses propres enjeux.

Introduction : La mort foetale in-utéro (MFIU) est un accident brutal et inattendu constituant un traumatisme inacceptable pour les parents. Sur le terrain, les soignants, notamment les sages-femmes, sont en première ligne dans la prise en charge des couples faisant face à la perte de leur enfant. Les étudiants sages-femmes peuvent également expérimenter au cours de leur stage, des annonces de MFUI et des accompagnements parfois rendus difficiles de situations de deuil périnatal. Quels impacts pourraient avoir le vécu en stage d'une situation de mort foetale in-utéro chez les étudiants sages-femmes de l'École de Sages-Femmes de Strasbourg ? L'objectif principal de ce travail était de mettre en évidence l'impact psychologique et professionnel d'une mort foetale in-utéro chez les étudiants sages-femmes.Matériel et méthodes : L'étude réalisée est de type semi-quantitative basé sur un questionnaire d'état des lieux, à but descriptif et de recrutement des volontaires pour participer à l'étude, et sur une analyse d'entretiens semi-directifs. Notre échantillon de population concerne les étudiants sages-femmes de l'École de Sages-Femmes de Strasbourg ayant rencontré une situation de mort foetale in-utéro en stage. Résultats : Un taux de participation au questionnaire d'état des lieux à hauteur de 55% a été observé. Ce questionnaire a permis de se rendre compte que la majorité des étudiants sages-femmes sont confrontés déjà très tôt, en troisième année de formation, à la mort foetale in utéro en stage, et davantage en service de salle de naissances. Les 8 entretiens menés ont permis également de constater que cette expérience est marquante pour ces étudiants. Face au décès périnatal, les étudiants sont confrontés à un conflit émotionnel intérieur et sont amené à changer leur comportement vis-à-vis des patientes. L'analyse des entretiens a d'ailleurs démontré une articulation centrale autour de la communication : la communication avec le couple, avec l'équipe mais aussi la communication pour partager son expérience et faire le ''deuil'' de l'expérience vécue en stage. Conclusion : Il existe un impact psychologique et professionnel pour les étudiants ayant vécu une situation de mort foetale in-utéro en stage. Bien que ces situations aient été difficiles à vivre, il en ressort systématiquement un apport positif.
