Introduction : l'incidence des douleurs chroniques post-césariennes (DCPC) est restée stable au cours de ces vingt dernières années malgré l'optimisation des techniques de prise en charge. En 2016, une méta-analyse de Weibel et al. estimait l'incidence de ces douleurs à 15% à 3 mois et 11% à 1 an. La réhabilitation améliorée après chirurgie (RAAC) est un programme englobant un ensemble de mesures pré-, per- et post-opératoires visant à améliorer la prise en charge chirurgicale, en favorisant le rétablissement précoce des capacités du patient après la chirurgie. De nombreuses études ont pu mettre en évidence son efficacité concernant la diminution de la durée de séjour, de la morbidité post-opératoire, de la consommation d'opioïdes en post-opératoire immédiat, des scores de douleur aiguë et du coût financier. Mais au-delà d'un bénéfice immédiat, l'impact d'un protocole de RAAC sur les douleurs chroniques n'a pas encore été évalué. L'objectif principal de notre étude était d'analyser l'incidence des douleurs chroniques post-césariennes programmées sous rachianesthésie dans le cadre d'un protocole de RAAC à 3 mois et 6 mois. Les objectifs secondaires étaient de préciser le type de douleur, d'évaluer son retentissement sur les activités quotidiennes et d'évaluer le taux d'adhésion au protocole de RAAC. Matériel et Méthodes : nous avons réalisé une étude préliminaire de faisabilité, descriptive, observationnelle, prospective, de cohorte sur une période de 13 mois, dans la maternité de niveau 2B de l'Hôpital Sainte Musse. Toutes les patientes ASA 1 et 2, programmées pour une césarienne sous rachianesthésie étaient incluses. Ce protocole mis en place en septembre 2017 au sein de l'hôpital n'a pas été modifié pour l'étude. Un questionnaire de douleur chronique, adapté du questionnaire de l'étude de L. Nikolajsen et al. était réalisé à 3 et 6 mois, lors d'un appel téléphonique. Pour les patientes présentant une douleur chronique, un question DN3 était ajouté. Le taux d'adhésion des équipes soignantes au protocole de RAAC était évalué à partir du recueil de 12 critères per et post-opératoires. Résultats : au total 131 césariennes programmées sous rachianesthésie ont pu être incluses. 131 patientes ont pu être contactées pour l'appel téléphonique à 3 mois et 97 à 6 mois. L'incidence des douleurs chronique de cicatrice de césarienne à 3 mois et 6 mois étaient respectivement de 14,5% et de 14,43%. Ces douleurs étaient majoritairement minimes au repos et modérées à l'effort et espacées de quelques jours. Chez les patientes présentant une DCPC, une douleur de type neuropathique (score DN3 ≥3/7) était retrouvée dans 68.4% des cas à 3 mois et dans 78.6% des cas à 6 mois. Le retentissement sur la qualité de vie se manifestait principalement par des difficultés à porter son enfant et certaines limitations à la mobilisation. Le taux d'adhésion au protocole de RAAC n'était pas significativement différent entre les patientes présentant des douleurs chroniques (64.47% à 3 mois et 60.71% à 6 mois) et celles sans douleur (62.95% à 3 mois et 63.96% à 6 mois, p = 0.601 à 3 mois et 0.330 à 6 mois). Conclusion : nos résultats sur l'incidence des douleurs chroniques post-césarienne programmée sous rachianesthésie ne diffèrent pas de ceux retrouvés dans la littérature, avant le développement de la RAAC. Les douleurs chroniques restent minimes à modérées, avec une composante neuropathique majoritaire et un impact modéré mais réel sur la qualité de vie. Ces résultats sont toutefois à interpréter en fonction de notre taux d'adhésion au protocole de RAAC. D'autres études sont nécessaires pour mieux préciser l'impact de la RAAC sur l'incidence des DCPC ainsi que pour tenter d'améliorer les protocoles de RAAC avec des éléments ciblés sur la prévention de cette douleur.
