Introduction : les troubles du sommeil représentent un enjeu important de santé publique touchant environ 30% des enfants. L'absence de prise en charge chez l'enfant avant l'âge de 8 mois conduit fréquemment à une persistance de ces troubles à l'âge de 3 ans. Le médecin généraliste (MG) se trouve, avec le pédiatre, en première ligne pour prévenir et dépister les troubles du sommeil du nourrisson dans sa première année de vie. Objectifs : l'objectif principal de ce travail de thèse a été d'évaluer la gestion des troubles du sommeil des nourrissons de 0 à 1 an par le MG dans son cabinet, et d'estimer dans un second temps les besoins en termes de supports matériels. Méthode : analyse quantitative et qualitative de données issues d'un questionnaire développé sur le logiciel Lime Survey à destination des MG installés en cabinet libéral. Resultats : parmi les 37 réponses au questionnaire obtenues, 76% des médecins disent systématiquement évaluer le sommeil des nourrissons. En première consultation, 90% des praticiens rappellent les règles hygiéno-diététiques et de bonnes pratiques. Trente-cinq pourcent des médecins recherchent une étiologie à la première vs 73% à la seconde consultation. Dans un premier temps, la cause organique est recherchée, surtout les troubles digestifs (59%), et dans un deuxième temps, les causes environnementales (22%). Les MG considèrent la pression des parents comme leur principale difficulté (78%). Puis, un manque d'outils de repérage ou diagnostiques (35%) et une méconnaissance des ressources existantes (30%) compliquent également leur prise en charge des troubles du sommeil. Cinquante-quatre pourcent ont l'habitude d'adresser les patients vers un médecin spécialiste. Parmi les outils proposés par le Réseau Morphée, un réseau de médecins et chercheurs en sommeil reconnu par la Société Française de Recherche et de Médecine du Sommeil (SFRMS), les MG s'intéressent avant tout aux fiches conseils (65%) et aux questionnaires d'évaluation du sommeil (43%). Conclusion : la question du sommeil est au cœur de nombreuses consultations du nourrisson en cabinet de médecine générale. Confronté à des difficultés, le médecin généraliste adresse le patient souvent directement vers un médecin spécialiste, alors qu'une prise en charge de premier niveau serait envisageable, notamment grâce aux outils d'aide existants sur des sites de référence. Il serait intéressant que les MG aient davantage connaissance de ces ressources, comme par exemple le Réseau Morphée, qu'il resterait à valoriser.
