Contexte : La peur et la douleur sont intimement liées dans le vécu du soin de l'enfant. L'avènement d'un outil d'auto-évaluation de la peur permet l'étude du lien entre ces deux émotions. Objectifs : L'objectif principal était de déterminer si l'auto-évaluation de la peur avait une influence sur l'auto-évaluation de la douleur. Les trois objectifs secondaires étaient de rechercher une corrélation entre score de peur et de douleur, analyser l'effet sur la consommation d'antalgiques et la satisfaction de l'outil. Méthodes : Un essai rétrospectif a été réalisé dans un CHU français comparant les valeurs des EVA chez les enfants âgés de 7 à 12 ans qui se présentaient aux urgences pédiatriques pour des motifs médicochirurgicaux avant et après mise en place d'une évaluation de la peur par le TrouillomètreÒ. Les scores de douleur sélectionnés ont été mesurés à l'aide de l'EVA. La consommation d'antalgiques a été comparée entre la période témoin et la période contrôle. La satisfaction a été évaluée à l'aide d'un questionnaire distribué à l'équipe paramédicale. Résultats : L'analyse a porté sur 154 patients dans le groupe après mise en place du TrouillomètreÒ et 154 dans le groupe de contrôle. Il n'y a pas eu de différence significative retrouvée entre les 2 groupes sur l'évaluation de la douleur mais une corrélation significative entre la peur et la douleur a été retrouvée (rho = 0,18, p-Value = 0,002). La consommation d'antalgiques de niveau 1 a augmenté et ceux de niveau 2 a diminué. Les équipes paramédicales étaient satisfaites de l'outil. Conclusion : Cette étude ne montre pas d'impact de l'évaluation de la peur sur l'évaluation de la douleur mais les analyses secondaires suggèrent qu'il existe un effet de la prise en charge de la peur sur la douleur déjà décrit dans la littérature et que le TrouillomètreÒ peut être utilisé pour mettre en évidence cette peur pour mieux la prendre en charge.

While working in Africa, anthropologist Julien Bonhomme encountered an astonishing phenomenon: people being accused of stealing or shrinking the genitals of strangers on the simple occasion of a handshake on the street. As he soon discovered, these accusations can have dramatic outcomes: the "sex thieves" are often targeted by large crowds and publicly lynched. Moreover, such rumors are an extremely widespread practice, having affected almost half of the African continent since the 1970s. In this book, Bonhomme examines the story of the "penis snatcher," asking larger questions about how to account for such a phenomenon--unique in its spatial and temporal scale--without falling prey to the cliché of Africa as an exotic other. Bonhomme argues that the public belief in sex thieves cannot be considered a superstition or form of mass hysteria. Rather, he brings to light multiple factors that explain the rumor's success and shows how the cultural dynamic can operate on a vast scale.
