Les restaurations esthétiques représentent aujourd’hui une part importante de l’exercice du praticien. Si l’adhésion à l’émail est aujourd’hui maîtrisée, l’adhésion à la dentine évolue sans cesse dans ses concepts. La composition et la structure histologique de la dentine rendent en effet plus dimcile une adhésion durable. Les adhésifs de quatrième génération, caractérisés par le mordançage total de Fémail et de la dentine et par la formation d’une couche hybride, représentent une étape décisive. Ces adhésifs présentent d'excellentes qualités, tant sur le plan de l’étanchéité et de l’adhérence que de la tolérance biologique. Seuls les protocoles restent difficiles à mettre en oeuvre, c’est ce qui justifie les dernières évolutions. En effet, les systèmes improprement commercialisés sous l’appellation “cinquième génération” proposent aujourd’hui une mise en oeuvre en deux étapes, tout en restant fidèles aux concepts de la couche hybride. Au vu des études encore peu nombreuses réalisées in vitro, ces adhésifs présenteraient les mêmes qualités que ceux de la quatrième génération, avec l’avantage d’une mise en oeuvre simplifiée. Cette approche semble indispensable afin de limiter les risques éventuels d’une manipulation inadaptée.
