Introduction : Les compétences des sages-femmes se sont étendues en 2009 avec la loi HPST leur permettant de réaliser le suivi gynécologique. Le nombre de médecins et de gynécologues en milieu rural ne cesse de décroître rendant difficile l'accès aux soins. Les sages-femmes jouent un rôle essentiel dans ce suivi de prévention et de dépistage. Méthode : L'objectif de cette étude était d'étudier les difficultés rencontrées, la qualité du suivi gynécologique des femmes vivant en milieu rural et leur opinion sur le rôle de la sage-femme. 12 entretiens ont été réalisés auprès de femmes effectuant leur suivi gynécologique chez une sage-femme, dans le département du Cher en 2020. Résultats : Les femmes interrogées ont déclaré des temps de trajet moyen pour se rendre chez le gynécologue de 40 kilomètres. Le médecin généraliste occupait une place importante dans le suivi gynécologique de prévention. Les femmes avaient consulté plusieurs professionnels de santé pour leur suivi. La régularité et la conformité des suivis gynécologiques des femmes interrogées étaient satisfaisantes. La majorité des femmes avaient rencontré une sage-femme pour la première fois lors d'un suivi de grossesse et avait initié ensuite un suivi gynécologique. Les femmes appréciaient chez les sages-femmes une approche plus centrée sur le patient et la possibilité d'un suivi génésique tout au long de leur vie affective et sexuelle. Discussion : La désertification des zones rurales par les médecins spécialisés impliquait des temps de trajet et des délais d'attentes plus longs et incitait les femmes à consulter davantage le médecin généraliste pour leur suivi gynécologique. Malgré les difficultés rencontrées, et un manque d'information, la régularité du suivi restait proche de celle observée au niveau national. La sage-femme a un rôle important à jouer dans la santé génésique de ces femmes, notamment en apportant une offre de soin de proximité et une prise en charge globale. Malgré l'augmentation du nombre de sages-femmes libérales, il reste des zones desservies où son statut manque encore de visibilité. Conclusion : La place de la sage-femme semble indispensable dans le milieu rural afin d'améliorer l'accès et la prise en charge du suivi gynécologique de prévention.

Contexte Le suivi gynécologique dépend d'inégalités territoriales et individuelles. Objectif : étudier la perception de femmes précaires vivant en milieu rural sur leur suivi gynécologique. Méthode Etude qualitative par entretiens individuels semi-directifs auprès de femmes précaires (niveau de précarité évalué par le score EPICES). Les lieux de recrutements étaient multiples. L'analyse du verbatim a été faite par théorisation ancrée. Résultats 14 femmes de 18 à 62 ans ont été rencontrées. Leur suivi médical était marqué par un faible recours voire un renoncement aux soins. Le suivi gynécologique était le reflet du suivi médical global. Les freins au suivi étaient la distance aux lieux de soins, le délais d'obtention des rendez-vous et la carence d'informations sur le dépistage du cancer du col de l'utérus. Le médecin généraliste n'était pas connu comme acteur du suivi. Conclusion Pour les femmes précaires en milieu rural, la délivrance d'informations adaptées à chaque patiente et la coordination du suivi gynécologique permettrait d'améliorer leur suivi.

En France, le suivi gynécologique des femmes est essentiellement assuré par les gynécologues et les médecins généralistes. Les buts de notre étude étaient de recueillir l'opinion des femmes sur leur suivi gynécologique afin d'évaluer l'activité gynécologique des généralistes de déterminer la répartition d'activité entre généralistes et gynécologues. Nous voulions aussi expliquer cette répartition en restituant les critères de choix des patientes et en analysant l'influence des facteurs socio-environnementaux. Cette étude a été réalisée auprès de 345 patientes issues de 33 cabinets de médecine générale d'Aquitaine. Notre travail a montré que la compétence du gynécologue était le principal argument pour le consulter préférentiellement. Pour le généraliste, la confiance et la disponibilité étaient les raisons les plus souvent évoquées. L'étude des facteurs socio-environnementaux a montré que les femmes généralistes réalisaient plus souvent le suivi de la ménopause et de la contraception que leurs collègues masculins. Parallèlement, un quart des patientes de notre échantillon préférait avoir un suivi par une femme. Les généralistes exerçant en milieu rural réalisaient significativement plus de frottis que leurs confrères urbains. Du fait de l'évolution de la démographie médicale, les généralistes seront de plus en plus amenés à participer au suivi gynécologique de leurs patientes. Ils devront, dans ce but, bénéficier d'une formation initiale à la gynécologie de qualité, s'investir dans la formation médicale continue, proposer plus souvent à leurs patientes d'assurer leur suivi, acquérir le matériel nécessaire, mais aussi convaincre les femmes de leur compétence dans cette discipline. Mais cela ne sera possible que s'ils ont l'envie et le temps de s'investir dans ce domaine.

These guidelines have been developed to support countries to relieve pain in their paediatric populations. These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain.
