Contexte : Un soin non programmé (SNP) correspond à une demande de consultation quel qu'en soit le motif, pour le jour même ou le lendemain, adressée au médecin généraliste aux horaires d'ouverture du cabinet. La gestion de ces soins est une problématique d'actualité qui serait en partie responsable de la saturation des services d'urgences en France. Objectif : Déterminer l'influence de la taille de la patientèle médecin traitant sur la gestion des soins non programmés au cabinet médical chez les médecins généralistes d'Auvergne Rhône-Alpes. Méthode : Un auto-questionnaire a été adressé par l'URPS - AuRA à tous les médecins généralistes de la régions Auvergne Rhône-Alpes dont il disposait d'une adresse mail en février 2021. Le questionnaire relevait les caractéristiques socio-démographiques des médecins généralistes, l'organisation de leur cabinet et enfin la quantité de soins non programmés effectués sur la semaine du 11 janvier 2021. Résultats : 220 questionnaires ont été analysés. La patientèle moyenne des médecins généralistes était de 1086 et ils effectuaient en moyenne près de 4 consultations de soins non programmés par demi-journée. Plus la patientèle des médecins est grande, plus le nombre de soins non programmés effectués était importante. L'absence de mode d'exercice particulier et une offre de soins en médecine générale insuffisance dans le territoire d'exercice était aussi associés à plus de SNP effectués. Conclusion : La taille de la patientèle d'un médecin généraliste influe positivement sur le nombre de soins non programmés effectués. Cependant il reste beaucoup de critères (connus ou non) qui influent aussi sur le nombre de SNP effectués.

Le médecin généraliste, par sa qualité de médecin traitant, est l'interlocuteur privilégié des patients en demande de soins non programmés. Leur densité médicale déclinant, le nombre d'admission pour consultations dans les structures d'urgences augmente chaque année. L'objectif de notre étude est d'évaluer les motifs d'admission des patients en demande de soins non programmés, ainsi que leurs éventuelles démarches en amont, afin de déterminer si leur venue est un choix consenti ou une contrainte, par défaut d'alternative ambulatoire, et de confronter ces résultats avec l'offre de soins disponible sur notre secteur. Pour cela nous avons interrogé les patients résidents de la ville d'Orange consultant spontanément aux urgences de l'hôpital d'Orange pendant les horaires d'ouverture des cabinets médicaux. En parallèle, les médecins généralistes et spécialistes de la ville d'Orange ont également été interrogés sur leurs disponibilités et accessibilités vis-à-vis des demandes de soins non programmés. 95 patients ont été inclus dans notre étude. Majoritairement jeune, de sexe masculin, et actif professionnellement, notre échantillon consulte surtout par choix aux urgences (68,4%), structure qu'ils jugent plus accessible (48,4%) et plus rapide (40%) qu'une prise en charge ambulatoire ; La disponibilité du plateau technique (40%), un motif d'admission traumatologique (46,3%), ou un sentiment de gravité de leur état (55,7%) sont également des motivations justifiant leur admission spontanée. 52,6% des patients n'ont tenté aucun contact médical libéral antérieur à leur arrivée. Les patients ayant essayé de joindre leur médecin traitant (34,1%) sont également ceux qui accepteraient plus volontier l'alternative d'une maison médicale de garde. Si l'on considère l'ensemble de la population médicale, des créneaux de consultations libres sont accessibles tous les jours de la semaine. Les médecins généralistes doivent faire face en moyenne à 3,5 demandes de consultations urgentes par jour ; ils sont satisfaits des relations qu'ils entretiennent avec les pécialistes (85,7%), mais sont plus de 70% à éprouver des difficultés à obtenir une radiographie. Une partie des patients se présentant actuellement spontanément aux urgences pourraient trouver une alternative intéressante en un planning commun de consultations libres réparties entre les médecins généralistes de la ville, ou en un centre de consultations non programmés, avec, pour les deux systèmes, un accès privilégié à des créneaux dédiés d'imagerie.

Ageing populations have gradually become a major concern in many industrialised countries over the past fifty years, drawing the attention of both politics and science. The target of a raft of health and social policies, older people are often identified as a specific, and vulnerable, population. At the same time, ageing has become a specialisation in many disciplines - medicine, sociology, psychology, to name but three – and a discipline of its own: gerontology. This book questions the framing of old age by focusing on the relationships between policy making and the production of knowledge. The first part explores how the meeting of scientific expertise and the politics of old age anchors the construction of both individual and collective relationships to the future.
