Ces 25 dernières années, des études en neurosciences cognitives de la musique ont mis en évidence une action de la musique et de la musicothérapie sur le fonctionnement cérébral, et notamment sur différents aspects de la cognition chez le sujet sain et pathologique. Dans la maladie d'Alzheimer, les troubles du langage se développent progressivement entravant la communication alors que certaines compétences musicales sont bien préservées, même à un stade sévère. Afin de mesurer l'efficacité de l'utilisation d'un support musical dans la remédiation des troubles du langage oral chez le sujet Alzheimer au stade modéré, nous avons réalisé une étude avec deux groupes, bénéficiant pour l'un d'une prise en charge en orthophonie en musique, et, pour l'autre, d'une prise en charge en orthophonie classique à raison d'une séance hebdomadaire sur une période de trois mois. Le langage spontané, les automatismes langagiers, le lexique ainsi que les habiletés discursives et pragmatiques ont été travaillés. Les résultats ont montré de meilleurs scores pour le groupe musical dans des épreuves étalonnées évaluant l'accès au stock lexical, la syntaxe, l'informativité du discours et la communication verbale. La combinaison de techniques de stimulation du langage à des techniques de musicothérapie pourrait constituer dans l'avenir une nouvelle approche plus efficace dans la prise en charge des patients atteints de la maladie d'Alzheimer.

Cette étude s'intéresse au rôle et à l'impact des centres d'intérêt dans la prise en charge orthophonique du langage oral, de la mémoire et de la communication des patients Alzheimer. Pour cela, une étude comparative a donc été menée en individuel chez des patients Alzheimer à un stade modéré et modérément sévère : durant trois mois, à raison de deux séances hebdomadaires, un groupe de patients a bénéficié d'une prise en charge dite « classique », un deuxième d'une méthode «personnalisée» axée sur leurs centres d'intérêt et un groupe « témoin » n'a bénéficié d'aucun suivi. Il apparaît que la prise en charge « personnalisée » a permis aux patients d'améliorer leurs capacités dans plusieurs domaines, tandis que les patients des deux autres groupes ont vu la plupart de leurs résultats se dégrader

La maladie d'Alzheimer (MA) touche de plus en plus de personnes. Elle entraine des troublesmnésiques, comportementaux et de la communication. Actuellement, aucun traitementmédicamenteux curatif n'existe. La Haute Autorité de Santé (2011) recommande les soins non médicamenteux comme l'orthophonie ou la thérapie par la musique pour améliorer le quotidien des patients et de leurs aidants. La prise en charge orthophonique a pour objectif le maintien des capacités de communication des personnes. La thérapie par la musique agit sur l'humeur, la mémoire, l'anxiété et le langage. Cette étude s'interroge sur les effets de la mise en place d'une introduction et d'une conclusion musicales en séance de rééducation orthophonique sur la communication de personnes atteintes de la MA à un stade modéré. 8 participants résidant en établissement hospitalier pour personnes âgées dépendantes ont participé à l'étude. Agés de 80 à 93 ans, ils présentaient un score au Mini Mental State compris entre 10 et 15. Ils ont participé à 8 séances individuelles de rééducation. 4 d'entre eux bénéficiaient d'écoutemusicale en début et en fin de séance, les 4 autres de séances sans musique. La grille d'évaluation des capacités de communication (GECCO) leur a été proposée en début et fin deprotocole. Les résultats chiffrés ne montrent pas d'effet significatif de la musique sur la communication. Une analyse qualitative de l'évolution de la communication des participants a été réalisée. Elle regroupe les observations issues de questionnaires aux soignants, le contenu des corpus de passation de la GECCO, et les observations des expérimentatrices. Cette analyse, en corrélation avec les études pré-existantes, met en évidence un effet de la musique sur le comportement, l'humeur, l'expression des goûts personnels et des souvenirs des participants. Il serait donc intéressant de tenir compte de ces résultats dans la rééducation orthophonique des personnes atteintes de la MA
