La Prise En Charge Médicamenteuse (PECM) du patient est un processus complexe qui fait intervenir de nombreux acteurs sur plusieurs étapes. Chaque étape peut comporter des risques qui peuvent eux-mêmes être à l’origine d’erreurs. C’est pourquoi les établissements de santé se sont actuellement engagés dans une démarche de sécurisation et d’amélioration de la qualité des soins. La pertinence et la continuité de la PECM des patients en sont des priorités. L’informatisation du circuit du médicament fait partie des éléments devant contribuer à cette sécurisation. Cependant, la mise en place de l’informatisation modifie profondément les modalités habituelles de prise en charge des patients, aussi bien au sein des unités de soins qu’au sein des pharmacies hospitalières. Nous avons alors étudié quels étaient ces impacts sur notre pratique pharmaceutique, afin de mieux appréhender les changements à réaliser et d’orienter l’évolution de la pharmacie clinique. Dans un premier temps, nous avons objectivé le ressenti des pharmaciens de l’équipe quant à l’augmentation du temps d’analyse pharmaceutique depuis la mise en place de l’informatisation au Centre Hospitalier Auban Moët d’Epernay (CHAME). Dans un deuxième temps, nous avons réalisé une étude rétrospective des interventions pharmaceutiques (IP) réalisées dans un service de médecine et de chirurgie, dans l’objectif de chercher des éléments permettant d’optimiser notre temps d’analyse centralisée. Nous avons étudié les types de problèmes retrouvés, leur classe ATC, leur origine, leur impact clinique selon l’échelle CLEO et nous leur avons attribué un index de navigation représentant le niveau de vigilance à apporter pour un patient. Cette étude nous a permis de mettre en place une feuille de route pour harmoniser nos pratiques de remplissage de notre dossier pharmaceutique. L’informatisation peut changer de manière bénéfique la prise en charge des patients, mais ces nouveaux outils doivent être utilisés de la manière la plus appropriée possible pour ne pas introduire de nouveaux risques et de nouvelles erreurs. Nous devons nous adapter à ces changements et apprendre à travailler avec. La pharmacie est à un tournant de son histoire et le pharmacien doit prendre l’informatisation comme un atout pour sa pratique clinique. Les technologies permettront au pharmacien de renforcer son rôle auprès du patient, mais également auprès des autres professionnels de santé.

La prise en charge médicamenteuse du patient est en cours d’informatisation via le logiciel Orbis® au Centre Hospitalier Universitaire de Toulouse. L’objectif de ce travail était d’évaluer son impact sur la sécurisation du circuit du médicament dans un pôle médico-chirurgical. Matériels et méthodes Nous avons accompagné le déploiement pour favoriser l’adhésion des équipes et harmoniser leurs pratiques. Un audit observationnel a été mené avant et après informatisation pour évaluer son effet sur la prescription et la traçabilité de l’administration. Les erreurs de prescription liées à l’informatisation ont été analysées via un recueil des interventions pharmaceutiques. Deux questionnaires évaluant la satisfaction des utilisateurs ont été diffusés dans les services. Résultats Sur 156 protocoles, 45 ont été obtenus après harmonisation et paramétrés sur le logiciel. 140 dossiers ont été audités. L'hospitalisation conduit fréquemment à des modifications dans la prise en charge médicamenteuse du patient. Aujourd'hui, la communication hôpital-ville, reposant principalement sur le compte-rendu d'hospitalisation, ne permet pas une transmission efficace de l'information pour assurer une prise en charge post-hospitalière précoce et optimale. L'hypothèse de recherche était que la réalisation pluriprofessionnelle collaborative, intégrant le pharmacien clinicien, de la conciliation des traitements médicamenteux de sortie, à travers la lettre de liaison, et sa transmission au médecin traitant par messagerie sécurisée améliorerait la continuité de la prise en charge médicamenteuse. Une étude prospective comparative a été menée dans 2 unités de soins du service de médecine interne du CHU Grenoble Alpes. Entre septembre 2017 et février 2018, 92 patients ont été inclus dans le groupe interventionnel et 97 dans le groupe contrôle. L'impact de la lettre de liaison a été évalué à partir du nombre moyen de modifications thérapeutiques réalisées en hospitalisation et poursuivies par le médecin traitant, dans chaque groupe, 3 mois après la sortie. Les résultats de cette étude ne mettaient pas en évidence de différence statistiquement significative mais montraient une tendance d'amélioration de la continuité de la prise en charge médicamenteuse 3 mois après la sortie dans le groupe interventionnel, avec une large satisfaction des médecins traitants. La transmission au médecin traitant de la lettre de liaison élaborée collaborativement et pluriprofessionnellement semble être un outil prometteur. Une large étude prospective multicentrique doit être conduite pour confirmer ces résultats.
