Until quite recently, the field of reproductive immunology was very much a neglected area of biology, seen by most reproductive physiologists as of only peripheral importance. It was generally acknowledged as curious that a female mammal tolerated the intrusion of alien sperm and the persisting presence of an alien fetus, while reserving the prerogative of rejecting grafts of tissue, even when these were from her own mate. Several theories were advanced to explain this paradox, each with some supporting evidence: all were eventually shown to be inadequate. And there the matter was, on the whole, permitted to rest. In the last few years, the situation has changed dramatically, and the neglected area of overlap between immunology and reproduction has again become densely populated by research workers. As a symptom of this resurgence of interest, a specialist journal (the Journal of Reproductive Immunology) has been launched to supply what had rapidly been perceived as a need.

The purpose of this book is to describe the nature of the materno-fetal immunobiological relationship and to suggest the direction in which the management of reproduction and its failure in man is moving. The several authors, who have written about their special fields of interest, need to be read within a framework designed to blend their contributions into a whole. This preamble provides a part of that framework, by describing the early development of the embryo, that of the placenta and its membranes and their anatomical relationship with maternal tissues: in other words, the stage upon which this materno-fetal dialogue takes place. Professor Maureen Young's 'tour de force', encapsulating the whole of fetal physiology into a single chapter, completes the background information. After Maureen Young's summary of fetal physiology, Matteo Adinolfi describes the development of the immune system in the fetus, including new information that allows more accurate speculation concerning the gestational age at which fetal immune responses of various kinds may begin. Charles Loke examines the nature of antigens which are found in the placenta, concentrating on those which occur on syncytiotrophoblast and suggesting roles for them in fetal development. Arnold Klopper covers the wide range of proteins and hormones which have been studied during pregnancy and found to vary in a potentially significant way. There is still no clear understanding of what causes the great majority of human congenital malformations. And since in most sorts of human disease and pathology that yet prevail prevention usually awaits understanding of cause, it is generally thought that the same is true of developmental aberrations. But is this true? For the relatively few congenital malformations whose causes are primarily environmental, it is plain that their discovery has enabled prevention, but not nec essarily immediately. It took a generation from the time of the discovery that maternal rubella was teratogenic to learn how to immunize against it. Much debate occurred before it was appreciated that thalidomide was a teratogen, and only its removal from the pharmacist's shelf and the end of the epidemic of limb defects attributed to the drug overcame the last doubts. For other proven environmental teratogens doubts and difficulties still con tinue. The claimed prevalence of fetal genital distortions due to female sex hor mones may have been exaggerated. Some potentially teratogenic therapeutic drugs, like anticoagulants, anticonvulsants, and anticancer chemicals, are still pre scribed despite this danger because of their benefits to pregnant women. For those congenital malformations whose basis is predominantly genetic or chromosomal it is different, however. Prevention has not been achieved by the discovery of such causes, as dramatic and revolutionary as some of them have been, except in the questionable sense of interference with reproduction by genetic coun seling or prenatal elimination. But this has not inhibited the romanticists.
