Immune Complexes and Human Cancer, the fifteenth volume of Contem porary Topics in Immunobiology, is a compilation of information derived from recent studies on the role of circulating immune complexes (CIC) in the patho genic manifestations of a variety of human cancers. Technical improvements in the detection of CIC in body fluids have resulted in data that indicate that CIC do occur in different types of cancer. In addition, tumor-associated antigens and antibodies have been detected in immune complexes of cancer patients' sera. Until recently the exact role and clinical relevance of immune complexes have been the subject of debate, partially because of the problems encountered in measuring immune complexes. But these problems are being confronted as more accurate measurement protocols are developed. Technical refinements, along with strict protocols, have provided evidence of heterogeneity in CIC, a factor that makes accurate detection of immune complexes in cancer patients difficult. Recent insights indicate that the measurement of immune complexes in cancer patients may be clinically useful not only as a tumor marker, but also in regard to the deranged immune response of tumor-bearing hosts and other disorders such as nephrotic syndrome, immune anemias, and clotting dysfunction.

In this issue, exciting new directions are outlined by fourteen groups of investigators working on critical areas in Breast Cancer Immunology. In the clinic, patients are responding to Her-2 peptides or GM-CSF transfected tumor cell vaccines. Futhermore, tumors under vaccine induced immune attack can prime the host to additional antigens. Selected chemotherapeutic agents are used to further vaccine efficacy. These promising results highlight the value of breast cancer immunotherapy. Although the clinical progress is exciting, significant challenges remain. Many tumor-associated antigens are self-antigens and vigorous measures will be required to induce consistent and sustained anti-tumor immunity. There is a pressing need for new immunotherapy targets. In this issue, the better-characterized glycoprotein antigens and novel molecules in angiogenesis are examined as new targets of breast cancer vaccines or immunotherapy. Continued effort in new antigen identification will be critical to cancer control. Finally, a reality check is warranted. Most breast cancer cells are still elusive to immune intervention.The mechanisms of such evasion are under intense investigation and much progress has been made. Alteration in antigen processing machinery is a major route of tumor evasion.

In the present grant we propose to explore the use of genetically engineered antibodies as therapeutic agents specifically attempting to augment and potentiate the host immune defense systems against breast cancer. We will use antibodies specific for HER2/neu, a molecule present on the surface of many breast cancers; its increased expression is associated with poor prognosis. To this antibody we will join the cytokines IL-2, IL-i 2, and GM-CSF. Expression of these cytokines by cancer cells has been shown to render them immunogenic. The anti-HER2/neu will be used to localize the cytokine at the tumor where it is expected to elicit an immune response. The resulting immune response would be expected to be specific not only for the targeting antigen, but also for other tumor associated antigens resulting in the destruction of both the tumor cells which express the targeting antigen as well as those that do not. Simultaneous targeting of more than one cytokine to the tumor would be expected to lead to synergism in immune activation and an even more potent immune response.

The elucidation of the immune response to cancer should be of great help in the development of new therapeutic strategies for the treatment of breast cancer. Based on recent advances in our understanding of antigen recognition by T lymphocytes, it has been possible to identify several tumor-associated antigens (TAA) recognized by CTLs. However, the expression for these TAAs has been shown to be relatively low in breast cancer tumor cells. A new protein named mammaglobin has been demonstrated to be exclusively expressed in the mammary epithelium. In addition, 90% of primary breast cancer tumors have high levels of expression of the mammaglobin protein. Given the exclusive mammaglobin expression in breast cancer tumors, this novel protein may prove to be a TAA highly specific for breast cancer that could be utilized in the near future for in vitro breast cancer-specific activation of CTLs. The discovery of mammaglobin-derived antigenic peptides that are highly expressed in breast cancer tumor tissue and are recognized by CTLs offer many exciting future therapeutic options for the treatment of breast cancer.

Heat shock proteins are emerging as important molecules in the development of cancer and as key targets in cancer therapy. These proteins enhance the growth of cancer cells and protect tumors from treatments such as drugs or surgery. However, new drugs have recently been developed particularly those targeting heat shock protein 90. As heat shock protein 90 functions to stabilize many of the oncogenes and growth promoting proteins in cancer cells, such drugs have broad specificity in many types of cancer cell and offer the possibility of evading the development of resistance through point mutation or use of compensatory pathways. Heat shock proteins have a further property that makes them tempting targets in cancer immunotherapy. These proteins have the ability to induce an inflammatory response when released in tumors and to carry tumor antigens to antigen presenting cells. They have thus become important components of anticancer vaccines. Overall, heat shock proteins are important new targets in molecular cancer therapy and can be approached in a number of contrasting approaches to therapy.
