In the past two decades we have witnessed very significant advances in the management of the very ill patient. The great success in this field of medical endeavour is largely due to the establishment of intensive care units, but a great deal of progress can also be attributed to the major developments in technology, which affect patient management and care as well as the many sophisticated techniques of diagnosis and patient monitoring. Imaging and Labelling Techniques in the Critically III covers this new important and difficult field of diagnosis and visual monitoring. By establishing the criteria and algorhythms for the choice of the different methods available for this purpose, defining the diagnostic signs on images and resolving some of the mis conceptions and pitfalls, this book will go a long way to help the reader, particularly those involved in the care of patients in the intensive care units. This book brings together many different methods of investigation and discusses the advantages and limitations of these techniques in different clinical circumstances. Some of the techniques are well established and their usefulness in the intensive care unit is in no doubt. Some of the newer techniques such as PET scanning or NMR imaging have not yet found a defined position of usage in the critically ill patient. There is, however, little doubt that in due course this situation will change.

Over the past two decades it has been increasingly recognized that whole-body ultrasound is an invaluable tool in the critically ill. In addition to offering rapid whole-body assessment, it has the advantage of being a bedside approach that is available at all times and can be repeated at will. Neuroimaging has developed rapidly, such as ultrasound, CT scanning, MRI, functional MRI, 7T MRI, and digital subtraction angiography, providing high-resolution acquisition and better contrast, making it easier to detect lesions and structural changes in brain diseases. Targeted diseases in neuroimaging include tumors, vascular diseases, neurodegenerative diseases, and psychiatric disorders, including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, epilepsy, severe depression, and schizophrenia. Nine years have passed since the first edition of Care of the Critically III Patient was published. As its editors Maurice Rapin and I were fortified by the many positive and helpful comments of reviewers and readers: the work appeared to have fulfilled our original aims and objectives. Intensive care medicine has, of course, advanced and changed in many ways since 1982 and it therefore seemed appropriate to consider the preparation of a second edition. Encouraged by the reception of the first and the timely prompting of the publishers we began, in 1986, to prepare for a new edition. Caring for critically ill patients is of universal concern and we were therefore delighted when Warren Zapol joined us as C'n editor widening our international base. The three of us subsequently spent many happy and productive times together planning the content, priorities, scale and contributions for the new edition. A great tragedy then overcame us: Maurice Rapin died suddenly. His death deprived us of a true friend and colleague who had been an international pioneer during the development of intensive care medicine. I will always remember, with affection and professional pride, the many hours he and I worked together in preparing the first edition of this book and also as founding editors of the now firmly established journal Intensive Care Medicine. An obituary, written by Warren Zapol, that appeared in the journal follows the Preface.
