The health care system is continuously changing and there is a new emphasis on quality of care and patient satisfaction, which are beginning to determine reimbursement amounts. With that said, the controversial topic of patient ratios per nurse has been talked about for years. Inadequate staffing of nurses or a high patient-to-nurse ratio will negatively influence patient care, outcomes and their satisfaction. High patient ratios will also lead up to nurse burnout and increased turnover rates. In the hospital acute care settings globally, nurses are caring for an average of eight patients to a minimum of two (Hinno, Partanen, and Vehvilainen-Julkunen, 2012). These high ratios are not conducive to give patients the care they deserve or the nurses their satisfaction of knowing they provided optimal care. So, the question of this study ask 0−́In adult hospitalized patients, does a lower patient-to-nurse ratio of 5:1 or less produce a better quality of care to patients compared to a higher patient-to-nurse ratio of 6:1 or greater?0+́ A literature review of 15 research articles was performed regarding the issue of high patient- to nurse ratios and the outcomes resulted from the studies. Because staffing and ratio issues are a concern in organizations, a proposal was set forth in an attempt to correct the problem at hand. California was one of the first states to implement mandated low patient-to- nurse ratios, which studies have shown increased patient satisfaction, improved patient outcomes, decreased nurse burnout and improved nurse retention and satisfaction (Aiken, Sloane, Cimiotti, Clarke, Flynn, Seago, Spetz and Smith, 2010). Mandated staffing ratios, as California has set in place and the implementation of an evidenced based staffing methodology are proposed solutions set forth by this writer. Implementation of this proposed solution will involve careful assessment, planning and communication efforts. To gain approval, the proposal will be submitted to administration with the current policy and the proposed policy for changes. Evaluation of the projects effectiveness, will be by the use of online surveys and graphs that show positive gain in patient and nurse outcomes as a result of a lower patient to nurse ratio. Educational tools will also be incorporated in the form of a power point presentation and surveys to assess for staff readiness for the new staffing changes. To inform others regarding the new staffing ratio changes, this writer will list the information of the organizations information page 0−́The Bridge0+́. The dissemination process will be ongoing until completion of the proposal. Staffing issues have the attention of many researchers; these researchers are looking for avenues to resolve these issues and assist in making the patients and the nurses satisfied and happy. The end of this article will have a complete literature review of fifteen studies and their results pertaining to nurse patient ratios.

The improvements of providing adequate staffing and quality patient care have been researched for many years. However, little has been done to solve the growing problem of high patient-to nurse ratios in hospitals. This raised the question: In hospitalized adult patients in acute care facilities, how does decreased patient-to-nurse ratio affect patient outcomes? Fifteen evidence based articles and journals were reviewed and the results were presented. Numerous studies showed direct association between nurse staffing and negative patient outcomes. The evidence proved decreased patient-to-nurse assignments and suitable working environment increased nurses' job satisfaction, lowered turnover, compassion fatigue, and lowered patient morbidity and mortality (Liang, Chen, Lee, and Huang, 2012). This paper presented an overview of the challenges nurses are faced to work with, high patient-to-nurse ratio, showing increased nurse-to patient ratio as a key solution to improve patient outcomes and healthy working environment for nurses. A pilot study will be conducted to compare the outcomes of nurses who were assigned to more patients, compared to nurses assigned to patients based on their acuity levels. Unbiased random selection will be used to pick three participants out of five RNs who work on a 12 hour day shift for the study. Data will be collected at the start and at the end of the 60-day trial period through a tool and questionnaires (Appendix A) from the participant group (A) and controlled group (B). The results from the pilot study will be presented to the key stakeholders for evaluation and approval of the proposed solution.
