Introduction: L'HPPI continue d' être la 1ère cause de décès maternels dans le monde et survient le plus souvent dans les pays en développement. Malgré l' existence de moyens de prévention les taux de mortalité maternelle restent élevés. Ainsi, il nous est apparu important de mener une étude pour savoir si les mesures de prévention préconisées .par l'OMS étaient bien suivies dans certains centres de santé en Côte d'Ivoire. Méthodes :Nous avons réalisé une étude prospective à la fois descriptive et analytique chez 120 patientes qui ont accouché au CHU de Cocody et 170 patientes à 1' HGAS, Sur une période de 3 mois, du 1"' Mai au 31 juillet 2019. Les données de la grossesse et de 1 'accouchement ont été analysées. Résultats :Les connaissances des médecins sont bonnes (95%), celles des sages-femmes sont moyennes (60 % ), et celles des utilisatrices sont à améliorer (30)0/o. La prévalence de l'HPPI était de 1,86% des accoochements au CHU de Cocody et de 0,64% à l' HGAS. Le profil des accouchées qui ont présenté une HPPI: 41% étaient âgées de moins de 25 ans, et 59% avaient plus de 35 ans. Elles étaient ménagères (38%) ou commerçantes (49%) sans instruction (52%) ou de niveau primaire (33%). Elles sont grandes multipares (61%), et ont négligé les CPN (74%). Elles ont été référées dans %des cas, de quartiers précaires (1 00% ), pour HPPI ( 100%) due à une atonie utérine (78%) ou à une rétention placentaire partielle (22%). Le traitement a consisté en un remplissage vasculaire par transfusion et macromolécules, révision utérine, massage utérin, antibiotique et injection d'ocytocine et de prostaglandine 250 mg en IM. Il y a eu 3 décès maternels. La prévention par la GATPA a été effective dans 92% des cas au CHU et 80% à l'HGAS, chez celles qui avaient des facteurs de risque.
