This volume explores in detail the molecular biology, genetics and immunology of the bacterium Helicobacter pylori that causes serious gastric diseases such as gastric cancer. Helicobacter pylori is an extremely successful pathogen that persistently colonizes the gut of about 50% of the world’s population. H. pylori and its human host share a long co-evolutionary relationship that dates back for at least last 100,000 years and possibly longer. Infection by this bacterium is a high-risk factor for the development of gastric diseases, including gastric cancer. Gastric cancer is associated with high morbidity and mortality and represents the 5th most common malignant tumour and the 4th leading cause of cancer-related death worldwide. H. pylori is the first bacterium that has been classified as a type-I carcinogen by the International Agency for Research on Cancer (IARC). Recent research progress identified crucial bacterial, host and environmental factors which control H. pylori-induced gastric malignancy. New studies also suggest that specific human germline mutations and other genetic aberrations have an important impact on H. pylori-induced pathology. In this volume, all these recently discovered mechanisms are reviewed in the light of gastric cancer development, and H. pylori epidemiology, virulence factors, immune evasion, pathophysiology, cancer signalling and novel therapeutic protocols are presented. Evaluates the carcinogenic risk to humans posed by infection with three schistosoma trematodes, three liver flukes and the bacterium Helicobacter pylori. These biological agents were selected for evaluation on the basis of evidence suggesting a causal association between infection and the development of human cancers. The first monograph assesses data on Schistosoma haematobium S. mansoniand S. japonicum, the three species which account for the vast majority of schistosomal disease in humans. The most extensive sections evaluate the results of studies of cancer in humans and experimental animals, concentrating on the strength of evidence linking infection with S. haematobium to squamous-cell carcinoma of the urinary bladder, and infection with S. mansoni or S. japonicum to an increased risk for cancers of the gastrointestinal tract. The monograph concludes that infection with S. haematobium is carcinogenic to humans and that infection with S. japonicum is possibly carcinogenic to humans; infection with S. mansoni could not be classified. The second monograph on liver flukes evaluates data on Opisthorchis viverriniO. felineus and Clonorchis sinensis. These foodborne trematodes establish a chronic infection of the bile ducts and more rarely the pancreatic duct and gall-bladder of humans and other mammals. Most carcinogenicity data centre on the possibility that these trematodes are involved in the development of cholangiocarcinoma or liver cancer. The monograph concludes that infection with O. viverrini is carcinogenic to humans and that infection with C. sinensis is probably carcinogenic to humans; infection with O. felineus could not be classified. The final monograph evaluates data on Helicobacter pylori. More than half of the world's population may be infected with this bacterium, which is responsible for most cases of chronic gastritis and duodenal ulcer. Citing several studies linking infection to gastric cancer in humans, the monograph concludes that infection with H. pylori is carcinogenic to humans.

Gastric cancer is the second most common cause of cancer mortality worldwide. Although the incidence of gastric cancer has declined over the past century in the United States, it remains one of the most common gastrointestinal neoplasms, particularly in immigrants and the socio-economically deprived. Recent years have seen major advances in our understanding of the pathogenesis of gastric cancer (especially regarding the importance of Helicobacter pylori and its associated inflammatory response) and the emergence of adjuvant oncologic therapies of proven benefit for advanced cases, in addition to surgery. As for gastric adenocarcinoma, elucidation of the underlying biology and molecular pathogenesis has led to much progress in the management of two other malignant gastric neoplasms, gastrointestinal stromal tumors and marginal zone B-cell (“MALT ) lymphomas.
