Physically active females, or female athletes, are at risk for the condition knownas the female athlete triad (Triad). The Triad is made up of three components that are distinctly separate, but intertwined: consuming inadequate energy for how much they are exercising, poor bone health, and problems with their menstrual periods. When female athletes don0́9t consume enough energy for how much they exercise, there can be hormonal imbalances, causing the body to conserve energy from some important functions including menstruation, and increased bone turnover resulting in a higher risk of stress fractures and early osteoporosis. Female athletes at risk for the Triad have ahigher risk of being injured, and don0́9t have the energy that they need to perform physically. To combat the Triad, we performed assessments to see how many female athletes were at risk for the Triad in Division I female athletes, and were surprised at the high number that were at risk for the Triad. We provided nutrition information and interventions aimed to decrease female athlete0́9s risk for the Triad. We also developed anew, quick method for female athletes, or busy individuals, to track their diet using the video feature on their cell phones. The benefits of providing nutrition information to the female athletes included: increasing their knowledge about the Triad, and things that they could do to prevent the Triad. The interventions did cause behavior change in some female athletes to increase how much energy they were consuming, decreasing their risk for the Triad.

After decades of work to identify and remove barriers to sports participation, female athletes are now participating in recreational and competitive sports at record numbers. However, female athletes are still underrepresented in sports research, leaving dangerous gaps in knowledge that we need to fill to encourage the development of more complete female athletes with better performance, healthier bodies, and improved mental health. Epidemiology provides a helpful framework for confronting these knowledge gaps and preventing them from undercutting the next generation of female athletes. It's urgent that female athletes are not treated as "smaller" male athletes, or worse--ignored altogether. By studying female athletes and addressing the unique physiological and sociological challenges they face, then translating the research into practical guidance and education, we have an opportunity to support generations of athletes to come. Every female athlete should have the tools to find their personal definition of "strong, " understanding how that individual and evolving strength will bring out their best in sports and in life. In this dissertation, I address pressing problems facing female athletes to evaluate new strategies for improving health and well-being by applying epidemiological principles and studies on research translation. The chapters in this dissertation intersect to cover six themes: (1) mental health; (2) bone health; (3) energy availability; (4) menstrual cycle; (5) performance; and (6) research translation (Figure 1-1). Chapter 1 introduces the scope of the field and how the themes intersect. In Chapters 2 and 3, we evaluate the effect of a nutrition education intervention on bone stress injury rates among female distance runners at two NCAA Division I institutions across a seven-year prospective study. In Chapter 4, we examine how a five-part video education series compares to online educational pamphlets for improving high school female runners' knowledge and for capturing their interest. In Chapter 5, we assess the risk of the Female Athlete Triad in ultramarathoners and complete exploratory analyses examining hormonal biomarkers and bone mineral density. And finally, in Chapter 6, we examine how early COVID-19 restrictions impacted the mental health and training of endurance athletes, which provides a broader context for how mental and physical health can be impacted by external factors, and how we can support athletes who are navigating prolonged breaks from competition. Recognizing and uplifting female athletes requires understanding their unique mental and physical health contexts, addressing the specific challenges they face, and empowering them to find their strong, in sports and in life.

The Female Athlete Triad (Triad) is a disorder comprised of three conditions: low energy availability, menstrual irregularity, and decreased bone mineral density. Triad prevalence is high among high school athletes, though their knowledge of the disorder is low. This study aimed to determine whether viewing a nutrition education video including Registered Dietitians describing Triad etiology, progression, and prevention, as well as testimonies of former college athletes who experienced the Triad would change knowledge of the Triad among female high school athletes. Ninety-three female athletes (15.89 ± 1.2 years) from four North Idaho high schools consented to participate. Following randomization to the intervention or control group, all participants completed a pre-survey with ten Triad knowledge questions highlighting Triad risk factors and consequences. Each question was answered using a Likert scale ("strongly disagree" to "strongly agree"). The intervention group (n = 46) then viewed the nutrition education video, while the control group ( n = 44) played a nutrition game in another room. Immediately after, all participants completed a post-survey containing the same ten Triad knowledge questions. Correct responses received one point; each participant received a score out of ten on each survey. Results of Mann-Whitney U tests revealed that pre-survey Triad knowledge scores between groups were similar, though post-survey scores were significantly higher among the intervention group as compared to the control (mean rank 64.07 vs. 26.09 respectively; U = 158.00; p
