Thirty-five years after its initial success as a form of technologically assisted human reproduction, and five million miracle babies later, in vitro fertilization (IVF) has become a routine procedure worldwide. In Biological Relatives, Sarah Franklin explores how the normalization of IVF has changed how both technology and biology are understood. The study of germ cells has undergone enormous advances in recent years and has entered into an explosive phase of new discoveries with the introd- tion of transgenic technologies and nuclear cloning. Basic knowledge and te- niques developed for lower vertebrate and invertebrate systems have facilitated the study of higher vertebrates, including humans. Many experiments that have first been performed on lower vertebrates provided the tools and strategies that could later be applied to other less readily available mammalian systems. The discovery of centrosomes in ascidians and sea urchin eggs now benefits st- ies of fertility and infertility in mammals including humans. External in vitro fertilization, now a common technique in assisted fertilization has only been possible as a result of numerous studies in lower systems in which external fertilization is natural. Egg activation, first explored in sea urchin and asc- ian eggs, now benefits cloning efficiency in farm and domestic animals. Gene manipulations and molecular methods have added to the possibilities of p- ducing live offspring with enormous biomedical, ecological, and economic implications. All sexually reproducing organisms produce primordial germ cells, a small population of cells that differentiate into gametes of either sex that carry to- potency, an ability to develop into an entire new organism. The two volumes on germ cells combine techniques in a variety of different systems and have selected those systems that have provided landmarks in advancing our kno- edge on germ cells.

While the organization of an international congress on in vitro fertilization (IVF) was an entirely new venture for us in 1983, since we had no experience but were full of optimism, we approached the organization of a second Congress in Vien na in 1986 much more calmly. Our experience had increased as had the readiness of many of our friends to help and of people interested in cooperating. However, would our Congress not lose its originality on account of the increased routine? This was certainly a possible danger which we wanted to prevent by means of two counter measures. The first concerned the scientific program: the new and trend-setting aspects of IVF would be discussed primarily rather than the results of traditional methods. The second measure concerned the social program, aimed more at private contacts in the intimate and familiar atmosphere of homes and workplace rather than meetings in luxurious but impersonal public institu tions. We believe that both these measures achieved the desired effect. Although we could not avoid some events taking place at the same time, the most important problems were treated and discussed jointly. Thus, the Congress was easy to survey, and all topics were thoroughly discussed.

It is widely understood that stem cell treatments have the potential to revolutionize medicine. Because of this potential, in 2004 California voters approved Proposition 71 to set up a 10-year, $3 billion program to fund research on stem cells. Under the direction of the California Institute for Regenerative Medicine, this program will pay to build facilities for stem cell research and will fund doctors and scientists to carry out research with the ultimate goal of helping to develop therapies based on stem cells. For this research to move forward, however, will require a steady supply of stem cells, particularly human embryonic stem cells. Those stem cells are collected from developing human embryos created from eggs-or oocytes-harvested from the ovaries of female donors. Thus much of the promise of stem cells depends on women choosing to donate oocytes to the research effort. The oocyte donation process is not without risk, however. Donors are given doses of hormones to trigger the production of more eggs than would normally be produced, and this hormone treatment can have various side effects. Once the eggs have matured in the ovary, they must be retrieved via a surgical procedure that is typically performed under anesthesia, and both the surgery and the anesthesia carry their own risks. Furthermore, given the very personal nature of egg donation, the experience may carry psychological risks for some women as well. With this in mind, in 2006 the California Institute for Regenerative Medicine contracted with the National Academies to organize a workshop that would bring together experts from various areas to speak about the potential risks of oocyte donation and to summarize what is known and what needs to be known about this topic. The Committee on Assessing the Medical Risks of Human Oocyte Donation for Stem Cell Research was formed to plan the workshop, which was held in San Francisco on September 28, 2006. This report is a summary and synthesis of that workshop.
