Cependant le dispositif peine à se mettre en place en Haute-Savoie, avec un faible recrutement de patients. L'objectif principal de l'étude était d'identifier les freins à la mise en place du dispositif en Haute-Savoie, l'objectif secondaire de proposer des solutions pour lever ces freins. Il a été réalisé une étude qualitative en focus groupes et différents freins ont été identifiés : le manque de formation des médecins, le manque de communication locale et générale au sujet du dispositif, l'absence de financement pérenne, l'absence de relais à la sortie du dispositif. Les solutions proposées ont été de créer des formations pour les médecins, d'améliorer la communication à travers des formations locales et générales, de créer des structures pouvant accueillir les patients sortant du dispositif et de trouver un financement pérenne par l'Assurance Maladie ou les mutuelles.

Introduction : La prise de conscience des pouvoirs publics concernant le poids de la sédentarité comme enjeu de santé publique a permis la mise en place du dispositif « sport santé sur ordonnance » permettant aux médecins généralistes (MG) de pouvoir prescrire de l'activité physique adaptée (APA) aux patients porteurs d'une ALD. Cependant la généralisation de ce dispositif est difficile à se développer en raison de nombreux freins identifiés dans des études précédentes. L'objectif principal de l'étude était d'évaluer les ressentis et les freins à la prescription d'APA chez les MG après avoir effectué une formation à la prescription. Les objectifs secondaires étaient de mettre en évidence les attentes concernant la mise en place d'actions sport santé et de comprendre pourquoi la somme de ces motivations n'abouti pas à un véritable maillage sport santé en France. Méthode : Il s'agit d'une étude descriptive qualitative, prospective, interrogeant les médecins généralistes ayant réalisés une formation à la prescription d'activité physique adaptée en ligne via le MOOC de l'IRBMS ou en présentielle via FMC-ActioN par questionnaire électronique envoyé de février à mai 2018. Résultats : 675 mails ont été envoyés avec un taux de réponse de 23.3%. Au total 96 MG répondaient aux critères d'inclusion. 79% considéraient cette formation nécessaire et 64% se sentaient aptes à prescrire de l'APA ,85% répondants convaincus du bénéfice de cette prescription après la formation mais peu ont prescrit. Pour optimiser la prescription 73% voudraient une consultation et une cotation spécifique. 57% disent avoir eu des refus de prescription en raison du manque de financement et 82% affirment que c'est un frein à leur prescription. 75% estiment ne pas être suffisamment informés des dispositifs régionaux « sport santé» avec 81% ayant du mal à trouver des structures d'accueils. Selon le MG 77% estiment que le ministère de la santé est l'institution la plus adaptée pour répondre à cet enjeu. Conclusion : De nombreux freins émergent de notre étude limitant la généralisation du dispositif sport santé sur ordonnance. Notre panel de MG sensible à la prescription de l'APA comme thérapie non médicamenteuse propose : D'accompagner les tous acteurs en renforçant la formation, de sensibiliser le public concerné à la thématique sport santé, d'optimiser le rôle des réseaux sport santé auprès des MG, de mettre en place un financement réaliste et pérenne.
