Foodborne diseases (FBD) are an important externality of agriculture and food systems, but only recently have they risen up the development agenda as the result of growing awareness of the health and economic burdens of FBD and how they relate to food systems with a focus on low-income and middle-income countries (LMIC), particularly African nations. The health burden of FBD is comparable with that of malaria, and over 90% falls on people in LMIC, with an economic burden of more than US$100 million per year. FBD have many other, less well-estimated effects on nutrition, gender, equity, and the environment. While understanding of food safety in domestic markets of LMICs has advanced greatly, risk management is in its infancy. This Research Topic will bring together leading regional perspectives on food safety in LMICs.

Ensuring the safety of food and the quality and safety of medicines in a country is an important role of government, made more complicated by global manufacturing and international trade. By recent estimates, unsafe food kills over 400,000 people a year, a third of them children under 5, mostly in low- and middle-income countries; every year poor quality medicines cause about 70,000 excess deaths from childhood pneumonia and roughly 8,500 to 20,000 malaria deaths in sub-Saharan Africa alone. The Federal Drug Administration (FDA) Office of Global Policy and Strategy is charged with improving capacity of the agency's foreign counterpart offices and increasing understanding of the importance of regulatory systems for public health, development, and trade. At the request of the FDA, this study sets out a strategy to support good quality, wholesome food and safe, effective medical products around the world. Its goal is to build on the momentum for strengthening regulatory systems and to set a course for sustainability and continued progress. The 2012 report Ensuring Safe Food and Medical Products Through Stronger Regulatory Systems Abroad outlined strategies to secure international supply chains, emphasized capacity building and support for surveillance in low- and middle-income countries, and explored ways to facilitate work sharing among food and medical product regulatory agencies. This new study assess progress made and the current regulatory landscape.

Animal products are vital components of the diets and livelihoods of people across sub-Saharan Africa. They are frequently traded in local, unregulated markets and this can pose significant health risks. This volume presents an accessible overview of these issues in the context of food safety, zoonoses and public health, while at the same time maintaining fair and equitable livelihoods for poorer people across the continent. The book includes a review of the key issues and 25 case studies of the meat, milk, egg and fish food sectors drawn from a wide range of countries in East, West and Southern Africa, as part of the "Safe Food, Fair Food" project. Food-borne disease remains a major public health challenge in Africa and Asia. Most of the foods that carry the highest pathogen risk are produced by smallholder farmers, marketed through the informal sector, and sold in wet markets. Given the significant role of informal markets in African and Asian food systems, attention is invested in understanding (1) how the people that participate in informal markets are exposed to risk, and (2) how they manage risk. We conduct a participatory risk analysis with a gender lens in 20 livestock and fish value chains to study whether gender-based differences influence risk of food-borne disease. We find that socially constructed gender roles are more important determinants of health risk than biological differences between men and women. Variations in risk exposure between men and women are mainly due to gender-based differences in occupational exposure, and secondarily to differences in consumption patterns. Women are important but under-recognized risk managers in the realms of food production, processing, selling, preparation, and consumption. Understanding the influence of gender on risk exposure and management is essential for improving food safety in informal markets.

Food systems are at a critical juncture—they are evolving quickly to meet growing and changing demand but are not serving everyone’s needs. Building more inclusive food systems can bring a wide range of economic and development benefits to all people, especially the poor and disadvantaged. IFPRI’s 2020 Global Food Policy Report examines the policies and investments and the growing range of tools and technologies that can promote inclusion.
