The National Institute for Occupational Safety and Health (NIOSH), an institute within the Centers for Disease Control and Prevention (CDC), is the federal agency responsible for conducting research and making recommendations for the prevention of work-related injury and illness. In fiscal year 1998, the Congress appropriated funds to NIOSH to conduct a fire fighter initiative. NIOSH initiated the Fire Fighter Fatality Investigation and Prevention Program to examine deaths of fire fighters in the line of duty so that fire departments, fire fighters, fire service organizations, safety experts and researchers could learn from these incidents. The primary goal of these investigations is for NIOSH to make recommendations to prevent similar occurrences. These NIOSH investigations are intended to reduce or prevent future fire fighter deaths and are completely separate from the rulemaking, enforcement and inspection activities of any other federal or state agency. The NIOSH Fire Fighter Fatality Investigation and Prevention Program conducts investigations of fire fighter line-of-duty deaths and injuries to formulate recommendations for preventing future deaths and injuries. The program does not seek to determine fault or place blame on fire departments or individual fire fighters but to learn from these tragic events and prevent future similar events. The Program Objectives are: Better define the characteristics of line-of-duty deaths among fire fighters; Develop recommendations for the prevention of deaths and injuries; Disseminate prevention strategies to the fire service. The enclosed investigation reports are examples of reports for traumatic injury and cardiovascular disease deaths of fire fighters in the last couple of years. Under its program, NIOSH investigators interview persons with knowledge of the incident and review available records to develop a description of the conditions and circumstances leading to the deaths in order to provide a context for the agency's recommendations.

The United States currently depends on approximately 1.1 million fire fighters to protect its citizens and property from losses caused by fire. Each year in the United States, approximately 100 fire fighters die in the line of duty. Sudden cardiac death is the leading cause of fatalities, followed by trauma. In 1998, Congress appropriated funds to the National Institute for Occupational Safety and Health (NIOSH) for a fire fighter safety initiative. As part of this initiative, NIOSH developed and implemented the Fire Fighter Fatality Investigation and Prevention Program (FFFIPP). The overall goal of the NIOSH FFFIPP is to reduce the number of fire fighter fatalities. To accomplish this goal, NIOSH conducts investigations of line-of-duty fire fighter deaths to identify contributing factors and to generate recommendations for prevention. This document summarizes the most frequent recommendations from the first 8 years of the NIOSH Fire Fighter Fatality Investigation and Prevention Program (FFFIPP). The overall goal of the program is to reduce the number of fire fighter fatalities. Through 2005, the FFFIPP investigated 335 fatal incidents involving 372 fire fighter fatalities. The investigations encompassed a variety of circumstances such as cardiovascular-related deaths, motor vehicle accidents, structure fires, diving incidents, and electrocutions. Fatalities have been investigated in career, volunteer, and combination departments in both urban and rural settings throughout the United States. This document shares the most common recommendations from the 335 investigations and more than 1,286 recommendations that were developed by NIOSH investigators. These recommendations were developed using existing fire service standards, guidelines, standard operating procedures, and other relevant resources over the first eight years of the program. Fire departments can use this document when developing, updating, and implementing policies, programs, and training for fire fighter injury prevention efforts.

Firefighters are called on to rescue people and protect property under serious and hazardous conditions. Some 100 firefighters die each year on duty, and another 95,000 are injured. Recommendations developed through the National Institute for Occupational Safety and Health (NIOSH) Fire Fighter Fatality Investigation and Prevention Program (FFFIPP) point to many safety practices that could improve the health and safety of the nation’s firefighters. The purpose of this report is to summarize the findings of a recent evaluation of the FFFIPP. Based on a national survey of fire departments and focus groups with frontline firefighters, the evaluation focused on firefighters’ awareness of FFFIPP recommendations, the extent to which recommendations are being implemented, the factors affecting implementation, and NIOSH’s dissemination practices. We learned that most fire departments are aware of the FFFIPP and follow its recommendations; nevertheless, various shortfalls, especially regarding cardiovascular health, motor vehicle safety, and procedures on the fireground, persist. The greatest shortfalls exist in small and volunteer fire departments where resources limit the ability to purchase needed equipment and prepare firefighters to carry out their duties safely. We identified numerous specific improvements that NIOSH could make to promote health and safety.

Of these fire fighters, approximately 336,000 are career and 812,000 are volunteers. The National Fire Protection Association (NFPA) and the U.S. Fire Administration estimate that on average, 90 to 100 fire fighters die in the line-of duty each year. In 1998, Congress recognized the need for further efforts to address the continuing national problem of job-related fire fighter deaths and funded NIOSH to implement a fire fighter safety initiative. With fire service stakeholder input, we developed the Fire Fighter Fatality Investigation and Prevention Program. The NIOSH Fire Fighter Fatality Investigation and Prevention Program (FFFIPP) conducts independent investigations of select fire fighter line-of-duty deaths. We do this to provide recommendations to prevent future deaths and injuries. The FFFIPP is a public health practice investigation program. We do not conduct our investigations to enforce compliance with state or federal job safety and health standards. We also do not determine fault or place blame on fire departments or individual fire fighters. Our program's goal is to learn from these tragic events and prevent future similar events. We do not investigate every fire fighter death. We have investigated approximately 40% of fire fighter deaths since the program's start in 1998. We prioritize fatality investigations using a decision flow chart, which is available on the FFFIPP website. Investigation priorities may change based upon the ongoing review of fatality data on leading risks to fire fighters and on fire service stakeholder input. Program Objectives: 1. Better identify and define the characteristics of line-of-duty deaths among fire fighters; 2. Recommend ways to prevent deaths and injuries; and, 3.
