Le@ système de soins et d’aide aux personnes âgées en perte d’autonomie en France se caractérise par un nombre importants d’acteurs. Cela entraîne des difficultés pour les personnes à trouver l’aide dont elles ont besoin dans un système fragmenté. Nous avons étudié deux des conséquences de cette fragmentation : l'utilisation inappropriée de l'hôpital, et le non-respect des droits fondamentaux de la personne au cours de l’institutionnalisation. Nos résultats mettent en évidence l'importance de raisonner à partir du domicile des personnes et en s’appuyant sur l’intégration de toutes les structures de soins et d’aide. Nous apportons également une réflexion méthodologique sur les études des filières de soins qui permettent d’appréhender ces phénomènes. Le constat de l’inadaptation du système de soins et d'aide crée une situation de tension qui pousse vers l'innovation. Le modèle PRISMA, par son antériorité, son efficacité prouvée au Québec, sa flexibilité démontrée offre l'opportunité d'étudier l'implantation d'un modèle d'intégration en France. L'étude des modifications organisationnelles et de pratiques professionnelles actuellement en cours permettra d’identifier les facteurs de mutation du système vers l'intégration. Cependant, le sens même de l'intégration est que la personne perçoive "le bon service ou bon moment au bon endroit par la bonne personne au moindre coût humain et social". Cet objectif nécessite le recours à un gestionnaire de cas, nouveau champ professionnel. Nous avons analysé au Québec en profondeur l’une de leur pratique professionnelle : la plan de services individualisé afin d'en préciser le concept, les règles de rédaction et les propriétés de l'outil.

Inclusion and Resilience has two broad objectives. The study seeks first to enhance knowledge about the current state of existing social safety nets (SSNs) in the Middle East and North Africa, assessing their effectiveness by bringing together new evidence, data, and country-specific analysis. Second, it proposes an open and informed debate on feasible policy options for making SSNs more effective and responsive to the population's needs. In order to do this, the authors illustrate how the main goals for SSNs-promoting social inclusion, better livelihood, and resilience to shocks-have already been achieved in some parts of the region, notwithstanding huge challenges. They also identify the groups that regional SSNs should make a priority: children and people living in rural and lagging areas. The study relies on newly collected data on citizens' preferences concerning redistribution and SSN design, discussing how political economy considerations could be taken into account in designing better SSNs, and proposing an agenda for reform, using global experiences and the new evidence presented in the book itself.
