IINTRODUCTION : Les fractures du col radial représentent 1% des fractures de l'enfant. L'embrochage centromédullaire selon Métaizeau a amélioré leur pronostic mais certains résultats restent mauvais. Plusieurs classifications déterminent le pronostic selon des stades. L'objectif est d'établir d'autres facteurs pronostics, dans une cohorte de patient où l'embrochage centromédullaire était utilisé, pour établir comment les mauvais résultats peuvent être améliorés.HYPOTHESES : Les stades IV, les types Jeffery II, les chirurgies ouvertes, les réductions insuffisantes, les lésions associées et l'âge supérieur à 10 ans sont de mauvais pronostic. MATÉRIEL ET MÉTHODE : 67 patients ont été inclus rétrospectivement : 39 filles et 28 garçons. 24 avaient un stade IV (36%), dont 2 types Jeffery II. 22 avaient une lésion associée (33%). 8 réductions isolées (11,9%), 58 embrochages centromédullaires (86,6%) dont 49 à foyer fermé (73,1%) et 9 à ciel ouvert (13,4%), et 1 une ostéosynthèse par vis (1,5%) ont été effectués. 10 réductions étaient insuffisantes, avec bascule supérieure à 20°. 22 avaient plus de 10 ans (33%). RÉSULTATS : Le recul moyen est de 77 mois, l'âge moyen de 15 ans. Les 11 mauvais résultats fonctionnels sont liés à un déficit en pronosupination. 16 radiographies finales sont anormales (23,9%), avec ossifications hétérotopiques dans 7 cas (10,4%), 5 nécroses (7,5%), 3 synostoses (4,5%) et 1 pseudarthrose (1,5%). Les 11 résultats fonctionnels moyens ou mauvais ont une radiographie finale anormale.
