L'insuffisance rénale chronique terminale, au sein d'une population vieillissante, verra sa prévalence augmenter au fils des années. Dans le but de pallier à la dégradation de la fonction rénale, un traitement de suppléance (dialyse ou transplantation) sera nécessaire dans la plupart des cas. Afin de choisir le traitement de suppléance le plus approprié, une évaluation gériatrique multidimensionnelle prétransplantation a été mise en place afin de prendre en compte les altérations du patient liées au vieillissement. L'intégration d'une expertise pharmaceutique clinique à cette évaluation a pour objectif d'identifier les problèmes liés à la thérapeutique. A ce jour, très peu d'études ont caractérisé la prise en charge thérapeutique de cette population âgée atteinte de maladie rénale chronique (MRC) avancée. Cette étude observationnelle avait pour objectif de décrire l'exposition médicamenteuse d'une population âgée atteinte de MRC avancée. Cette exposition était illustrée par la consommation de médicaments potentiellement inappropriés (MPI), selon la liste européenne « EU(7)-PIM » ; et de médicaments possédant des propriétés anticholinergiques et/ou sédatives objectivés par la mesure du Drug Burden Index (DBI). Les patients inclus dans l'étude étaient adressés par leur néphrologue dans le cadre d'une évaluation gériatrique multidimensionnelle prétransplantation au sein de l'hôpital de jour de l'hôpital des Charpennes (Institut du Vieillissement, Hospices Civils de Lyon). Parmi les 139 patients inclus dans l'étude, au moins un MPI était prescrit chez 74,1% des patients. Les principaux MPI identifiés étaient : les inhibiteurs de la pompe à protons, les alpha-1-bloquants et les antihypertenseurs centraux. Concernant l'exposition aux médicaments anticholinergiques et/ou sédatifs, la prévalence d'exposition était de 79,9%. La mise en évidence des principaux MPI et médicaments à propriétés anticholinergiques et sédatives délétères dans cette population spécifique permettra de sensibiliser les pharmaciens et les médecins (gériatres et néphrologues) dans un but d'optimisation de la prise en charge médicamenteuse (déprescription) et de prévention de l'iatrogénie médicamenteuse.
