As we approach the 25th anniversary of the first recognition of HIV/AIDS in 1981, this book reflects on the international impact of the disease. It has persistently remained a global issue, with more than 50 million people worldwide estimated to have been infected since that date. This ambitious book, written by 165 authors from 30 countries, offers a multi-country comparative study that examines how the response to the common, global threat of HIV is shaped by the history, culture, institutions and health systems of the individual countries affected. Increasingly the shift of health systems has been from prevention only as the main containment strategy, to a strategy that includes scaling up HIV treatment, and care and prevention services, including antiretroviral therapy. Thus, all parts of the health system must be involved; policy makers, healthcare professionals and users of the services have been forced to think differently about how services are financed, how resources are allocated, how systems are structured and organized, how services are delivered to patients, and how the resulting activity is monitored and evaluated in order to improve the effectiveness, efficiency, equity and acceptability of the response. This book is unique in attempting to describe and assess a range of responses across the globe by situating them within the characteristics of each country and its health system. Most chapters combine a health policy expert with an HIV specialist, allowing both a 'top down' health system approach and a 'bottom up' HIV-specific perspective. There are thematic and analytical sections, which provide an overview and some suggestions for solutions to the most serious outstanding issues, and chapters which analyse specific country and organisational responses. This study documents the results to which the World Bank's Multi-Country AIDS Program (MAP) financing in Africa has contributed over the last five years ("What has the MAP achieved?"). It uses extensive and detailed data from surveys and national HIV and AIDS programs from 30 MAP countries that are not usually publicly available or captured in routine World Bank reporting systems. It introduces a new Results Scorecard and Framework for better measuring and reporting on results of Bank-financed HIV/AIDS programs in Africa in the future. The book shows that the MAP has dramatically increased access to HIV prevention, care and treatment across Africa. MAP funding has supported children orphaned by AIDS, prevented mother-to-child transmission, helped countries build capacity for scaled up, more effective national responses to HIV and AIDS, including providing treatment. As the HIV/AIDS epidemic has grown to become the fourth biggest killer in the world and the leading cause of death in sub-Saharan Africa so the pressure on those working in the field to expand the scale of their activities has increased. Non-governmental organizations (NGOs) have been especially influential in the response to HIV in developing countries and the pressure to increase the scope and impact of this work is particularly strong. "Making an Impact in HIV and AIDS" recognizes that scaling up NGO programmes requires more than just additional resources or the straightforward replication or expansion of interventions. The book analyzes when expansion is appropriate, how to make it effective, how to measure the costs, and what the implications for organizations may be. It examines these issues through the experience of NGOs working in different contexts and in all aspects of HIV/AIDS including prevention, care and support, and mitigating the impact of the epidemic. This volume examines how local actors respond to Africa’s high dependence on donor health funds. It focuses on the large infusion of donor money to address HIV and AIDS into Malawi and Zambia and the subsequent slow-down in that funding after 2009. How do local people respond to this dynamic aid architecture and the myriad of opportunities and constraints that accompany it? This book conceptualizes dependent agency, and the condition in which local actors can simultaneously act and be dependent, and investigates conditions under which dependent agency occurs. Drawing upon empirical data from Malawi and Zambia collected between 2005 and 2014, the work interrogates the nuanced strategies of dependent agency: performances of compliance, extraversion, and resistance below the line. The findings elucidate the dynamic interactions between actors which often occur “off stage” but which undergird macro-level development processes.
