L’incidence en France de l’accident vasculaire cérébral est de 130 000 cas par an, dont 40 000 décès et 30 000 handicaps lourds. Dans tous les cas, il existe des conséquences à moyen et long terme sur l’autonomie des patients. Après une prise en charge neurologique et si besoin, en médecine physique, la sortie du monde hospitalier est souvent vécue comme compliquée. Malgré cette difficulté, il n’existe que peu de recommandations pratiques à destination du médecin généraliste concernant cette étape, hormis les recommandations de prévention secondaire. L’étude effectuée concernant 20 patients suivis en Médecine Physique et Rééducation fonctionnelle du CHU de Caen, consiste à mesurer l’évolution de différents facteurs pouvant influer sur l’autonomie du patient.Un lien significatif a été retrouvé concernant l’âge, l’efficience cognitive, l’autonomie durant l’hospitalisation et la spasticité. Les résultats orientent également vers un rôle important de la fatigue, la dépression et les douleurs, notamment d’épaule. Le dépistage puis le suivi régulier des symptômes mis en évidence dans cette étude semblent donc nécessaire. La mise en place d’outils de communication dès la phase hospitalière permettrait d’améliorer le relai avec l’équipe multidisciplinaire ambulatoire, coordonnée par le médecin généraliste, afin d’améliorer l’autonomie du patient.
