La mise en place d'une réforme, par la loi du 18 janvier 1994, permet la création d'Unités de consultations et de soins ambulatoires (U.C.S.A.) à l'intérieur des prisons, afin d'améliorer l'accès aux soins des détenus. Cependant, cette réforme est difficilement applicable, notamment à la maison d'arrêt de St Michel, du fait de la vétusté des locaux. L'auteur tente, à travers ce travail, de montrer comment la création d'une nouvelle prison (la maison d'arrêt de Seysses), et donc d'une nouvelle U.C.S.A., permet de se rapprocher des objectifs de cette réforme, tout en soulignant les points noirs persistants dont les relations entre le personnel soignant et le personnel pénitentiaire.

La prise en charge sanitaire des personnes détenues représente un véritable enjeu de santé publique qui a connu une évolution majeure avec la loi du 18 janvier 1994 relative à la santé et à la protection sociale. En effet, cette dernière a confié cette mission à l'hôpital public et permis la création d'unités de consultations et de soins ambulatoires (UCSA), de véritables services hospitaliers au sein des établissements pénitentiaires. 25 ans après la mise en place de cette réforme, nous avons effectué des journées d'observation à la maison d'arrêt de Corbas et au centre pénitentiaire de Saint-Quentin Fallavier au cours de l'année 2017 et 2018 afin de décrire le fonctionnement et l'organisation de la pratique de la chirurgie dentaire en milieu carcéral. Grâce à la création d'une base de données concernant les rendez-vous et le volume d'actes réalisés dans les deux services, nous avons tenté de réaliser une analyse de l'offre ainsi que de la consommation de soins au sein des UCSA des prisons de Lyon.

Résumé : Depuis la loi du 18 janvier 1994, les soins des détenus sont confiés aux structures hospitalières, ce afin d'établir une relation de confiance entre un patient, qui n'a pas le choix de son praticien et un professionnel, indépendant de la structure punitive. La population carcérale est une population particulière qui présente de nombreux facteurs de risques. Elle a un besoin en soins plus important que la moyenne de la population. Une législation, aussi bien mondiale, européenne, que nationale est mise en place pour répondre au mieux à ces besoins spécifiques. En France, les services hospitaliers dans les centres pénitentiaires permettent de faciliter l'accès des détenus aux soins. A la Maison d'Arrêt de Strasbourg, un cabinet dentaire est installé dans l'Unité de Consultations et de Soins Ambulatoires. Tous les types de soins y sont réalisés, même si les soins d'urgences et les soins conservateurs en représentent la plus grande partie. L'objectif de ce travail est de détailler, dans un premier temps, la législation et la déontologie existant sur le sujet, ainsi que les différentes études publiées et, dans un second temps, les différents facteurs de risque et la situation à l'UCSA de Strasbourg, concernant les soins odontologiques.

La pratique odontologique en milieu carcéral se fait au sein des Unités de Consultation et de Soins Ambulatoires (UCSA). La particularité de cet exercice tient à la fois à l’environnement lui-même, et à la population rencontrée, non représentative au niveau sociodémographique, et médical du reste de la population française. Une étude rétrospective a été réalisée sur l’ensemble des détenus rencontrés au cabinet dentaire de l’UCSA de la maison d’arrêt de Reims durant l’année 2009. Les trois quarts souffraient de pathologies somatiques et/ou psychiatriques, parmi lesquels les troubles mentaux et du comportement, et en particulier les troubles addictifs aux substances psychoactives prédominaient. La consommation en examens médicaux ou en consultations spécialisées y était élevée. L’analyse de l’activité odontologique montre une proportion élevée des consultants en urgence et une prédominance des soins postérieurs dont une part importante d’avulsions. Une évaluation de l’état de santé bucco-dentaire de la population carcérale nécessiterait la réalisation d’une étude prospective sur l’ensemble des entrants afin de mieux cerner le profil odontologique des détenus dès leur arrivée ainsi que leurs antécédents et leurs besoins en soins

While the use of the analyst’s own reveries in work with patients has increased in recent times, there has been little critical inquiry into its value, and the problems it may lead to. The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie?
