L'acteur principal de la promotion du médicament est la force de vente, présente sur le terrain : les visiteurs médicaux, les délégués pharmaceutiques, et les visiteurs hospitaliers. Mais parce que les produits concernés impactent la santé de la population, la régulation de la promotion du médicament ne peut se permettre d'être similaire à celle des autres produits de consommation. Cette thèse présente la genèse et l'évolution de l'encadrement de la promotion du médicament. Elle se concentrera sur l'impact de l'affaire Médiator(r) sur les organismes et les régulations encadrant la Visite Médicale, mais aussi sur la profession en elle-même, qui a vu ses effectifs fortement décroître au cours des années qui ont suivi cette crise. Ces effectifs ont également été impactés par la crise du COVID-19, par la forte réduction des activités de terrain. Conjointement à ces crises, avec l'arrivée des nouvelles technologies et méthodes de formation, et l'émergence de nouveaux métiers de communication, une question se pose : celle de la légitimité et de l'avenir des métiers de promotion de terrain dans l'industrie pharmaceutique.
