The Health Transition Fund Project on First Nations and Inuit Home Care was carried out over an 18-month period in five sites encompassing eight First Nations of geographical & cultural diversity. Tasks carried out during the Project included conducting a community needs assessment, developing a service delivery plan & the necessary infrastructure, and implementing the home care services identified in the plan. This report presents findings of an evaluation of the Project carried out using a combination of both qualitative and quantitative data collection methods. The findings relate to the need for home care, the validity of the national First Nations & Inuit home care framework, the integration & co-ordination of home care services with the adult care program, and critical development issues such as communication, staffing, training, and supportive infrastructure. Appendices include descriptions of home care program models used in the First Nations communities studied.
