Le circuit de la nutrition parentérale (NP) pédiatrique en milieu hospitalier est un processus complexe au cours duquel toutes les étapes peuvent être génératrices de risques. L'administration des mélanges de NP, étape ultime, nécessite une attention particulière. L'objectif principal de notre étude est d'évaluer dans un hôpital pédiatrique la conformité des pratiques d'administration des NP aux recommandations du rapport de l'IGAS de 2015. Nous avons réalisé une étude descriptive observationnelle et prospective sous la forme d'un audit clinique. Le recueil des critères a été réalisé à l'aide d'une grille, renseignée lors de l'observation des manipulateurs pendant la préparation et le branchement des poches. Au total, 78 administrations ont été incluses. Ces observations nous ont permis de définir des axes d'amélioration prioritaires, et ont mis en évidence la nécessité de rédiger une procédure d'administration des NP en pédiatrie validée par le CLAN. La comparaison aux recommandations de l'IGAS a permis de mettre en évidence des points forts : l'absence d'adjonction de médicaments non nutritifs dans les poches (100%), le contrôle de l'identité du patient sur la poche (100%), la traçabilité du lot de la poche administrée (100%), le branchement en binôme (82%). D'autres critères doivent être améliorés : le contrôle de la poche administrée en regard de la prescription (88%), la vérification de l'intégrité (18%), de l'aspect (1%) et de la péremption des poches (51%). La traçabilité des contrôles effectués était absente pour toutes les observations. Le suivi des températures et la gestion des alarmes des réfrigérateurs ne sont pas conformes.
