Introduction : en général bénigne, la gastroentérite (GEA) est l'un des premiers motifs de consultation pédiatrique selon Sentinelles. En 2013-2014, les enfants de 1 à 4 ans étaient les plus touchés (10 312/100 000 hab.). Le taux d'hospitalisation était de 0,4%. La diarrhée aiguë peut entrainer une déshydratation et menacer le pronostic vital. La perte de poids est le principal facteur d'évaluation de la déshydratation. Le travail des généralistes peut être difficile et s'éloigner des recommandations pour des difficultés pratiques. L'objectif est de décrire les pratiques des médecins généralistes de Gironde sur la prise en charge de la GEA de l'enfant de moins de six ans et d'identifier les difficultés rencontrées avec les SRO. Méthode : l'étude reposait sur des questionnaires prospectifs standardisés, distribués à des généralistes de Gironde, entre le 1er janvier et le 30 avril 2016. Résultats : un échantillon de 34 médecins a inclus 143 enfants (âge moyen : 38 mois). 9 enfants ont été adressés à l'hôpital et les autres ont bénéficié d'une prise en charge ambulatoire. Les données sur la fréquence des selles et vomissements sont les moins recherchées (46 et 41% des cas). Les autres données sont recherchées dans 100% des cas. L'orientation initiale des patients a respecté les recommandations dans 90% des cas. Des consignes de surveillance et de diététique sont données à plus de 90% des parents, mais incomplètes. Chaque item n'apparait que dans 50% des cas environ. Une fiche conseil n'est donnée que dans 12% des cas. Les médecins jugent le SRO seul, inefficace dans 47% des cas, et le pensent mal donné dans 56% des cas. Effectivement 22% des parents n'ont pas donné le SRO prescrit, et seulement 58% le jugent efficace. Conclusion : des progrès restent nécessaires pour un allégement des prescriptions médicamenteuses, qui passeront par une meilleure utilisation du SRO. Une fiche d'explications pourrait diminuer les difficultés rencontrées, mais à ce jour l'ensemble de cette démarche est jugée trop chronophage.

Ce travail de recherche s'intitule “Etude descriptive du niveau de connaissance et de compétences concernant la prise en charge de la gastro entérite du nourrisson et de l'enfant des internes de médecine générale en stage de médecine ambulatoire” Il évalue et compare les internes en premier stage de médecine ambulatoire de la maquette de formation “SN1” aux internes de derniers stages de médecine ambulatoire “SASPAS”. L'évaluation et la comparaison concerne les habitudes et pratiques de prise en charge de la gastro entérite du nourrisson et de l'enfant. Celle-ci est réalisée par le biais d'un questionnaire établi sur la base des recommandations de sociétés savantes. Après quelques questions permettant d'évaluer les affinités et expériences de chaque étudiant, 21 questions concernant différents aspects pratiques, épidémiologiques, thérapeutiques ou diagnostiques sont soumises à chaque participant. Ce questionnaire a ensuite été proposé à des internes de toute la France par le biais d'internet, et les données ont été collectées et traitées pour établir un score pour chaque étudiant. Une moyenne a finalement été établie pour chacun des groupes comparés. 89 participations ont été recueillies et analysées, dont 60 dans le groupe SASPAS et 29 dans le groupe SN1. Le groupe SASPAS a obtenu une moyenne supérieure au groupe SN1 statistiquement significative avec un score global de 15.025 contre 14.142 pour le groupe SN1 (pValue 0.020) Secondairement, la population SASPAS se sent significativement plus “apte” à la prise en charge de la GEA du nourrisson et de l'enfant avec une moyenne de 7,1 contre 6,2 pour le groupe SN1 (pValue 0,01) selon l'auto évaluation de chaque étudiant. Donc les étudiants de fin d'internat sont meilleurs et plus confiants concernant la prise en charge de la gastro entérite du nourrisson et de l'enfant.

Second, all of the outcomes examined in this book are influenced by factors other than population growth. Moreover, the impact of population growth may itself vary according to the presence or absence of other factors. This again makes bl anket statements about the effects of population growth difficult. Throughout the chapters, the authors try to identify other relevant factors that influence the outcomes we discuss or that influence the impact of population growth on those outcomes.
