Introduction : la couverture vaccinale contre les HPV reste insuffisante en France. Depuis 2019, la vaccination anti-HPV par Gardasil 9 est étendue aux garçons de 11 à 19 ans. L'objectif principal de notre étude était d'évaluer les connaissances des 14-18 ans sur la vaccination anti-HPV. Méthode : il s'agissait d'une étude épidémiologique, observationnelle, descriptive, monocentrique. Les données ont été recueillies à l'aide d'auto-questionnaires anonymes. Résultats : parmi les 621 adolescents participants, 20% ont déjà eu un rapport sexuel et 36% déclaraient être vaccinés contre les HPV. 77% savaient que la transmission des HPV se fait par rapport sexuel et le cancer du col de l'utérus est la complication la plus citée. Les adolescents connaissaient l'existence du vaccin anti-HPV à 76%. 48% connaissaient les nouvelles recommandations de vaccination et 22% le nombre d'injection. Discussion : dans notre étude, les filles semblent mieux informées que les garçons. Est-ce parce qu'elles sont la cible de la vaccination depuis plus longtemps que les garçons ? Des études qualitatives sur les freins à la vaccination des garçons seraient utiles pour ensuite les comparer à ceux des filles. On aurait pu s'attendre à un intérêt pour les supports numériques mais les adolescents déclarent préférer l'école ou le cabinet médical comme lieux d'information. Conclusion : dans notre étude, une faible proportion des adolescents connait l'extension de la vaccination aux garçons et le nombre d'injections. L'existence du vaccin comme moyen principal de protection est bien connue. Le déploiement de sessions de prévention scolaire en lien avec les médecins généralistes serait pertinent.

Introduction : L'infection à Papillomavirus Humains (HPV) est une infection sexuellement transmissible (IST) fréquente, responsable de lésions bénignes, mais également de pathologies cancéreuses chez l'homme et chez la femme. La prévention repose principalement sur la vaccination anti-HPV, recommandée chez les filles et les garçons jusqu'à 19 ans révolus. Les jeunes hommes sont donc également concernés par les risques liés à ce virus et par la vaccination anti-HPV. Notre objectif était d'évaluer les connaissances qu'ont les jeunes hommes de 18 à 26 ans sur ce virus et sur la vaccination anti-HPV mais également d'étudier les moyens d'information reçus et les facteurs pouvant influencer leurs connaissances. Matériel et méthode : Nous avons réalisé une étude quantitative observationnelle à partir d'un questionnaire effectué téléphoniquement entre le 4 mars 2022 et le 28 juillet 2022. Les participants ont été recrutés par l'intermédiaire de médecins généralistes lors d'une consultation. Résultats : Sur 129 appels téléphoniques effectués, 91 ont abouti à la réalisation du questionnaire. L'échantillon était principalement composé d'étudiants de 21 à 23 ans, vivant en milieu urbain. 5,5 % étaient des hommes ayant eu des relations sexuelles avec des hommes (HSH) et 8,8 % exerçaient une activité dans un domaine de la santé. Parmi l'ensemble des répondants, 67 % avaient déjà entendu parler des HPV. Parmi eux, 80 % évoquaient une infection mixte et la majorité a décrit une transmission possible par voie sexuelle. La proportion de personnes infectées ainsi que le taux de cancers et de décès liés à HPV étaient le plus souvent sous-estimés. De plus, les pathologies décrites comme étant en lien avec un HPV étaient principalement des pathologies féminines comme le cancer du col de l'utérus ou le cancer du vagin. La vaccination anti-HPV était connue pour 65,9 % des participants. Les réponses concernant les recommandations vaccinales étaient variables. Les sources d'information les plus souvent décrites sont les proches ou les recherches sur internet. La totalité des répondants estimaient ne pas avoir suffisamment d'informations sur ce virus. Discussion : Les connaissances qu'ont les hommes sur les HPV et la vaccination anti-HPV sont faibles, malgré les nouvelles recommandations vaccinales incluant les garçons. Ce manque de connaissance est cohérent avec la littérature. L'intérêt porté à ce virus est probablement d'autant plus faible qu'ils n'ont pas conscience de la fréquence des maladies induites et qu'ils sous-estiment son impact sociétal. L'information sur les HPV mérite donc d'être poursuivie et intégrée aux messages de prévention destinés à la jeunesse au même titre que les autres IST. Conclusion : Il reste encore de nombreuses améliorations à apporter afin d'informer les jeunes hommes, par exemple via les médias, la scolarité, des affiches mettant en scène des femmes et des hommes mais également via les professionnels de la santé jouant un rôle primordial en matière de prévention.
