Contexte : la dépression est une pathologie fréquente, invalidante et insuffisamment diagnostiquée par les médecins généralistes. Les preuves d'efficacité de la Formation Médicale Continue (FMC) pour améliorer la prise en charge de la dépression sont mal évaluées. Objectif: évaluer l'impact d'une FMC interactive sur l'identification de l'Episode Dépressif Caractérisé (EDC) par les médecins généralistes. Méthode : essai d'intervention contrôlé randomisé par médecin. L'analyse en intention de traiter a porté sur les données de 35 médecins volontaires. Les données recueillies de façon continue ont été extraites des fichiers médicaux informatiques des médecins. L'intervention a consisté en une séance de FMC de type didactique et interactive d'une durée de 16 heures délivrée en deux jours. La mesure de l'effet de la formation a été faite par comparaison d'un groupe d'intervention (n=18) à un groupe témoin non formé (n=17). Le critère de jugement principal était la proportion de médecins qui avaient augmenté leur taux d'identification de la dépression d'au moins 50% (effet positif) 3 mois après la randomisation. Pour évaluer la validité des diagnostics, nous avons mesuré la proportion des diagnostics déclarés qui étaient confirmés par le MINI (Mini International Neuropsychiatrie Interview). Résultats : nous avons observé un effet positif chez 8 médecins du groupe FMC (44%) et 5 médecins du groupe témoin (29%). Cette différence de 15% n'était pas significative (p=0,28). La validité des diagnostics était bonne 93%. Elle s'est améliorée après l'intervention dans le groupe FMC (79% vs 86% ; p=0,l) et dans le groupe témoin ( 92% vs 100% ; p=l). Conclusion : la FMC lorsqu'elle fait appel à des interventions simples utilisées isolement n'a pas d'effet sur l'identification des patients déprimés par les médecins généralistes. Les réseaux de santé pourraient constituer une alternative pour assurer une meilleure prise en charge des patients dépressifs.
