Introduction : Afin de répondre efficacement aux nouveaux besoins de soins de la population dus au vieillissement démographique, le Centre hospitalier universitaire (CHU) de Nice à mis en place dans son service d’accueil des urgences (SAU) des infirmières de coordination spécialisées en gériatrie (IGU) afin d’améliorer la prise en charge de ses patients âgés de 75 ans ou plus. Nous avons tenté d’évalué l’impact de cette nouvelle organisation en conduisant une étude rétrospective de type avant / après. Cette dernière compare deux groupes de patients : un constitué de patients évalués par les IGU et un groupe contrôle. Notre critère de jugement principal était le taux de reconsultation à sept jours. Les résultats n'ont pas permis de mettre en évidence un effet positif des IGU à la lumière de notre critère de jugement principal mais suggèrent d’autres apports bénéfiques sur la prise en charge de nos patients âgés. Ainsi, leur prise en charge par les IGU est associée à une augmentation des aides à domicile et des évaluations cognitives, une augmentation de l’implication des familles dans la prise en charge et enfin une augmentation du suivi gériatrique par des consultations à distance. Cette nouvelle façon de procéder s’inscrit dans une dynamique qui est soutenue par l’état via les Agences régionales de santé (ARS), plébiscitée dans la littérature scientifique et semble aller dans la bonne direction pour l’évolution de nos services d’urgence. De futures études pourraient évaluer d’autres aménagements de nos services comme l’adaptation des locaux aux personnes âgées, la création d’unité d'hospitalisation de courte durée (UHCD) gériatriques, ou autres coopérations avec des équipes de gériatrie au sein des urgences dans l’optique d’améliorer et de fluidifier la prise en charge de tous nos patients.
