Many noteworthy advances in our knowledge of the pericardium, its functions and diseases and their relation to heart failure have been made since the first edition of this book appeared in 1981; and no other book that covers in detail the physiology and pathophysiology has since been published. The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.

Cells are suspended in media of different Ca+ con 2 2 tent whose Ca+ concentration is monitored by a Ca+ -selective electrode. When the cells are lysed (by digitonin) there is an upward or downward deflection of the elec 2 trode signal. Introduction : Le syndrome coronaire aigu est une pathologie fréquente et grave, et reste un diagnostic parfois difficile à poser. Depuis 1995, un registre national est dressé tous les 5 ans afin de mieux comprendre et d’évaluer la prise en charge de ce syndrome : USIK 1995, USIC 2000 et en 2005 le registre FAST-MI. Notre étude a pour but de comparer le résultats du service de Cardiologie de l’hôpital R.BALLANGER d’Aulnay-sous-Bois, à ceux retrouvés dans le registre national FAST-MI, notamment en terme de délais de prise en charge, conformément aux Recommandations françaises et européennes. Matériel et Méthode : Etude prospective, observationnelle, unicentrique, menée du 1er au 31 Octobre 2005, réalisée dans le service de Cardiologie d’Aulnay-sous-Bois, menée parallèlement au registre FAST-MI. Population divisée en quatre groupes : groupe A : SCA avec sus ST, à troponine + groupe B1 : SCA sans sus ST, à troponine + groupe B2 : SCA sans sus ST, à troponine – groupe C : suspicion de SCA Mesures et critères de jugement basés sur les délais de prise en charge : latence patient : premier symptôme – premier contact avec le système de soins latence intervenants médicaux : premier contact avec le système de soins – revascularisation délai de prise en charge global : premier symptôme – revascularisation. Résultats : 24 patients inclus, dont 15 parallèlement au registre FAST-MI ; 8 dans le groupe A, 7 dans le groupe B1, 6 dans le groupe B2 et 3 dans le groupe C. Selon les Recommandations de la Conférence de Consensus de 2006, 5 patients du groupe A ont eu un délai de prise en charge global inférieur à 90 minutes, après calcul des délais porte à porte-cardio et porte cardio-ballon. Discussion : En 10 ans, la prise en charge du SCA a évolué majoritairement dans le sens de l’angioplastie coronaire : 13 % en 1995, 33 % en 2005. La thrombolyse a diminué puis s’est stabilisée, passant de 38 % en 1995 à 27 % en 2005. La mortalité a nettement diminué, passant de 8,3 % en 1995 à 4,3 % en 2005. Dans le centre étudié, les objectifs de délai de prise en charge sont atteints et aucun décès n’a été comptabilisé. A noter, le caractère unicentrique de l’étude et la petite taille de la population reste une limite et un biais important. Conclusion : Grâce à ces études menées tous les 5 ans, un état des lieux précieux est fait sur le SCA, permettant de revoir les délais de prise en charge, les traitements et les pronostics. Le réflexe de l’appel du centre 15 est primordial en cas de douleur suspecte de SCA, le plus important est de minimiser les temps de prise en charge, et la coordination entre les différents acteurs de la chaîne de soins, que sont les urgentistes et les cardiologues, reste essentielle.
