Les traumatismes crâniens (TC) sont I 'une des premières causes de mortalité et de handicap chez l'enfant et représentent à ce titre un problème majeur de santé publique. Après un TC sévère, les séquelles sont fréquentes et peuvent toucher divers domaines : épilepsie, séquelles motrices, cognitives (troubles du langage, troubles visio-spatiaux, troubles de l'attention, de la mémoire et des fonctions exécutives, notamment). Elles peuvent être immédiates ou émerger de manière différée, notamment les troubles cognitifs. Les troubles du comportement sont également rapportés assez fréquemment chez les enfants victimes de TC sévère. Peu d'études ont évalué les troubles du comportement chez I 'enfant à distance d'un TC sévère, et leur évolution longitudinale sur plusieurs années. Les objectifs de ce travail étaient : (1) d'évaluer le comportement à 7 ans de la survenue d'un TC sévère survenu dans l'enfance, à l'aide d'un questionnaire évaluant les comportements internalisés et externalisés (formes pédiatriques et adulte du questionnaire Child Behavior Checklist, CBCL/ ABCL) au sein de l'étude prospective longitudinale « Traumatisme Grave de I 'Enfant » (Cohorte TGE) ; (2) d'évaluer les facteurs (démographiques, environnementaux et de sévérité du traumatisme crânien) associés aux mesures standardisées du comportement ; (3) d'évaluer les associations de facteurs concurrents (mesurés à 7 ans du TC) avec les mesures du comportement ; (4) enfin, d'évaluer l'évolution longitudinale des mesures de comportement pour les patients ayant préalablement été évalués avec le même questionnaire à 3, 12 et 24 mois du traumatisme crânien. Méthodes : L'étude longitudinale TGE a porté sur des patients âgés de 0 à 15 ans, consécutivement admis sur une période de trois ans dans un même centre de réanimation neurochirurgicale, dans les six premières heures d'un TC sévère accidentel (score de coma de Glasgow et / ou Injury Severity Score > 16) et a débuté à l'hôpital universitaire Necker-Enfants Malades de Paris 5, où tous les patients éligibles ont reçu un traitement conforme aux directives internationales pour la prise en charge des traumatismes sévères (Carney et al., 2003) dans l'unité de soins intensifs neurochirurgicale de ce centre régional de traumatologie pédiatrique. Le suivi s'est déroulé dans le service de rééducation des lésions neurologiques acquises des hôpitaux de Saint-Maurice, où la grande majorité (83 0/0) des patients éligibles ont reçu une réadaptation multidisciplinaire après les soins aigus et des évaluations standardisées dans le cadre du suivi prospectif longitudinal. Sept ans après le TC, les troubles du comportement ont été évalués dans les 2 groupes à l'aide de l'auto- et de l'hétéro-évaluation par les parents du questionnaire CBCL/ ABCL, qui génère huit échelles de syndrome et de problèmes de comportement, ainsi que trois scores composites de problèmes d'internalisation, d'externalisation et de problèmes totaux. Les données recueillies comportaient également le contexte sociodémographique (sexe, niveau d'éducation des parents), la gravité initiale du TC (GCS, ISS, durée du coma) et les évaluations spécifiques concurrentes [mesures objectives : niveau de handicap global (Glasgow Outcome Scale-Extended), type de scolarité en cours, efficience intellectuelle ; mesures recueillies par questionnaires : fonctionnement exécutif, participation, qualité de vie, fatigue et fonctionnement familial]. Lors de l'évaluation à 7 ans, un groupe contrôle a été inclus, étroitement apparié aux patients pour l'âge, le sexe et le niveau d'études des parents. Résultats : les auto- et les hétéro-questionnaires ont mis en évidence significativement plus de problèmes de comportement en ce qui concerne le comportement agressif et les troubles de la pensée, 420/0 et 360/0 des scores de comportement externalisés se situant au-delà du seuil clinique pour les problèmes de comportement externalisés. Concernant les auto-questionnaires, les patients avaient également significativement plus de difficultés de type repli sur soi/ dépression, plaintes somatiques et comportement intrusif. Les parents rapportaient de plus des difficultés significatives en ce qui concerne le comportement contraire aux règles, l'attention et les problèmes sociaux. Les troubles du comportement externalisés rapportés par les parents étaient significativement associés à la sévérité initiale du traumatisme crânien. Enfin, pour le sous-groupe (n=20) de patients qui avaient eu des évaluations à 3, 12 et 24 mois du TC, les troubles du comportement avaient tendance à augmenter au fil du temps pour les comportements extériorisés (sur les 8 participants dépassant le §euil clinique 7 ans après la blessure, 5 participants étaient en dessous du seuil 3 mois) et les problèmes totaux (5 participants sur 10).
