Contexte : La douleur chronique est définie comme évoluant depuis plus de 3 à 6 mois, et susceptible d'affecter péjorativement le comportement ou le bien-être du patient, attribuable à toute cause non maligne. La prévalence de la douleur chronique oscille entre 10,1 et 55,2% selon les études. 31,7% en France. Cette étude a cherché à explorer la prise en charge des douleurs chroniques et leurs représentations en médecine générale. Méthode : Une étude qualitative, par entretiens semi-dirigés individuels a été menée auprès de généralistes volontaires, exerçant dans le secteur de Saint-Etienne. Une analyse thématique de contenu a été effectuée sur la base des retranscriptions et sur les notes prises par le chercheur lors des entretiens à l'aide du logiciel OpenOffice®. La validité interne a été établie par une double analyse indépendante. Résultats : Médecins et patients recherchent un diagnostic, support supposé à une acceptation de la maladie y compris par la société. Malgré le suivi des recommandations les consultations sont décrites comme complexes. Certains médecins font part de leur sentiment d'impuissance et parfois même de contre attitude. Les solutions envisagées passent par la formation à la communication et par la création de référents-douleurs accessibles. Conclusion : Les participants au-delà de l'approche biomédicale mettent en évidence la complexité du patient douloureux chronique. Une approche globale du patient sous tendue par la relation médecin malade leur paraît un gage de meilleure prise en charge.

In The New Ecological Order, Luc Ferry offers a penetrating critique of the ideological root of the "Deep Ecology" movement spreading throughout the United States, Germany, and France. Traditional ecological movements, or "democratic ecology," seek to protect the environment of human societies; they are pragmatic and reformist. But another movement has become the refuge both of nostalgic counterrevolutionaries and of leftist illusions. This is "deep ecology." Its followers go beyond practical critique of human greed and waste: they call into question the very possibility of human coexistence with nature.
