En France, une grossesse sur trois est non programmée. Dans deux tiers des cas elle survient chez des femmes qui déclarent utiliser une contraception au moment du rapport fécondant. Elle justifie dans deux tiers des cas un recours à une IVG. Nous avons comparé deux populations de femmes en demande d'une IVG à 28 ans d'intervalle dans le service d'orthogénie du centre hospitalier universitaire et au centre de planification et d'éducation familiale de Caen de décembre 2014 à juin 2015. Les patientes se présentant à la première consultation IVG ont pu répondre par écrit à une cinquantaine de questions concernant leur profil sociodémographique, leurs pratiques contraceptives et le contexte de l'IVG. Comparativement à 1987, le statut marital apparaissait plus stable, les femmes étaient plus diplômées et nombreuses en activité professionnelle. Elles étaient également plus nombreuses à s'estimer suffisamment informées sur la contraception. Les femmes aux antécédents d'IVG étaient moins nombreuses en inactivité professionnelle. En 2015, l'IVG était réalisée plus souvent chez des femmes sous méthode contraceptive et sous contraception médicalisée. La contraception orale était la première mise en cause en 2015 (les méthodes naturelles en 1987). Le nombre jugé satisfaisant d'enfant (comme en 1987) puis les problèmes financiers et/ou de logement (les raisons professionnelles en 1987) avaient motivé la demande d'IVG. Le recours à l'IVG en 2015 reste important. Il reste majoritairement secondaire à un échec contraceptif. Des efforts sont à fournir auprès des professionnels concernés et du public en matière d'apprentissage et d'information sur la contraception, la sexualité et l'IVG.
