Après avoir rappelé la description de l'oreille interne, expliqué les définitions des barotraumatismes (BT) et des accidents de désaturation (ADD), l'auteur met en évidence un lien entre la présence de symptômes vestibulaires et le diagnostic d'ADD et de BT pour des symptômes cochléaires. À l'aide d'une étude rétrospective entre 2004 et 2014, l'auteur a comparé les données de 138 patients, répartis selon le diagnostic retrouvé. L'étude s'est portée sur les caractéristiques de la population, celles de la plongée, symptômes manifestés, examens complémentaires initiaux, finaux et traitements. L'auteur compare ses résultats avec ceux de la littérature. Il retrouve une latéralité droite prédominante dans l'atteinte vestibulaire lors d'ADD en rapport avec la présence d'un foramen ovale perméable fréquemment retrouvé lors d'ADD. Il conclut en discutant la place d'autres examens complémentaires dans le diagnostic de ces accidents et sur l'urgence quant au traitement de l'ADD.

Introduction : les accidents de plongée sont une cause de lourds handicaps chez des sujets jeunes en bonne santé. La démocratisation de ce sport entraine une modification des profils des victimes. Une étude des accidentés permet de mieux organiser leur prise en charge. Méthode : une étude rétrospective monocentrique sur 6 ans a regroupé les dossiers traités au centre de médecine hyperbare de l'hôpital sainte marguerite à Marseille. Une étude épidémiologique descriptive et des modalités de pris en charge pré hospitalière est conduite. Résultats : les accidentés sont majoritairement une population masculine (75.8%), jeune (44 ans) avec un Indice de Masse Corporelle de 24.6 +/- 0.4 et pour 77% non-fumeurs. Dans la majorité des cas ils n'ont pas d'antécédents ni de traitements en cours. Chez les plongeurs loisir, les plus accidentés sont de niveau III (42.7%) et chez les professionnels se sont les MF1 (33%). Conclusion : les accidents de plongées touchent une population jeune en bonne santé. Lors de la rédaction du certificat de contre-indication des situations à risque doivent être identifiées. La prise en charge des accidents est variable. La mise en place de registres nationaux doit permettre l'analyse des facteurs de risque et l'uniformisation des prises en charge.
