Iodine is an essential micronutrient and an integral component of the thyroid hormones, which are required for normal growth and development. The iodine deficiency disorders (IDD) encompass a spectrum of adverse health effects including goiter, cretinism, hypothyroidism, growth retardation, and increased pregnancy loss and infant mortality. This volume summarizes the current understanding of the effects of iodine deficiency as well as iodine excess. It also discusses best practices for salt iodization, the mainstay of global IDD prevention efforts, and other forms of food fortification. The effectiveness of iodine supplementation for vulnerable populations, an evolving strategy in many regions, is also described. Low level environmental exposure to chemicals such as perchlorate and thiocyanate, which competitively block thyroidal iodine uptake, appears to be ubiquitous worldwide. This provision is a governmental, economic and social problem. Apart from this, however, the scientific and medical problem of iodine deficiency and its effect on brain development and function is one of great interest and importance for developmental neurology and psychology. Even though the specific preventative agent is known, we do not totally understand the neurobiological questions raised.

The disorders induced by iodine deficiency affect at least one billion people. Because ofits effects on brain development, iodinedeficiency is the single most preventable cause of mental retardation in the world. Therefore, the United Nations and the Heads of State of almost all the world's countries represented at the Summit for Children in 1990 adopted resolutions to eradicate the disorders induced by iodine deficiency (IDD) by the year 2000. For geological and socio-economic reasons, most of the populations affected by iodine deficiency disorders live in isolated and usually mountainous areas, in pre industrialized parts ofthe world. The problem of iodine deficiency in Europe has been greatly underestimated in the last decades. After the remarkable studies on the effects of iodine deficiency and their prevention and correction in Switzerland, IDD was generally considered no longer a significant public health problem in Europe. However, surveys carried out in the early 1980's under the auspices of the European Thyroid Association, clearly demonstrated the persistence of moderately or even severely affected areas. These surveys also highlighted the lack ofinformation about large parts ofEurope, especially its eastern part. It is only quite recently, following major changes in international relations and thanks to the support of UNICEF, WHO, the International Council for the Control of Iodine Deficiency Disorders and the European Thyroid Association, that more extensive surveys have been conducted in several parts of Europe hitherto almost unexplored. These surveys showed that most European countries were iodine deficient.

This book addresses the causes and effects of nutrient deficiencies along the cell-to-communities continuum. The book is primarily concerned with a lack or deficiency of one or more micro- or macronutrients in connection with malnutrition, under nutrition, and starvation. Embedded within the deficiency states is acute restriction whereby food is withdrawn completely for short periods, as when individuals are adhering to religious requirements or undergoing surgical procedures. Further downstream is the consumption of a fraction of the normal diet, as when individuals are dieting or when there is restriction in the amount or variety of food available. The causes of such reductions in dietary intake are varied and also include the social context of poverty, financial limitations, and famine. Refugees and displaced persons may also be vulnerable to under nutrition or total starvation. Diseases may also impact on the total food consumed, such as when there are physical impediments (intestinal obstruction or dysphagia) or anorexia (induced by organic disease or as a disease process per se, ie, anorexia nervosa). This book, organized as approximately 125 chapters in 17 major sections, covers the variable manifestations of dietary restrictions on cells, whole organs, the individual, and societies.
