Contexte : La fréquentation aux urgences pédiatriques en France est en constante augmentation année après année. Diverses raisons ont été avancées et notamment les symptômes souvent bénins et fréquents tel que la fièvre mais perçu par les familles comme nécessitant des soins urgents. Dans ce contexte, nous avons évalué le devenir à court terme d’enfants ayant consulté aux urgences, pour essayer d’identifier les facteurs influençant les consultations itératives rapprochées. Méthode : Etude observationnelle, prospective dans le service des urgences pédiatriques de l’hôpital Robert Debré. Après tirage au sort, les dossiers médicaux des 501 enfants inclus de moins de 6 ans ont été étudiés et un entretien téléphonique a été réalisé avec leurs familles. Résultats : Sur les 501 enfants inclus dans l’étude, la fièvre est le principal motif de consultation (26,5%) devant la gêne respiratoire (18,8%) et la toux (17%) ; 27% avaient déjà consulté dans les 8 jours précédents pour le même motif et 18,5% ont re-consulté dans le système de soin primaire dans les 8 jours suivant leur passage aux urgences. Ainsi, sur les 15 jours englobant le passage aux urgences, 39% des enfants ont consulté au moins 2 fois, dont 10,5% 3 fois et plus. La fièvre était présente de façon plus importante (61,5% versus 44%, p=0,0009) chez les enfants ayant déjà consulté avant leur venue aux urgences, et n’avait aucun impact quant aux re-consultations. Conclusion : Les consultations médicales des enfants aux urgences pédiatriques sont associées dans 39% des cas à une autre consultation médicale proche. La fièvre, motif fréquent de consultation, semble liée à ce phénomène de consultations multiples rapprochées.

Objectif : Connaître le devenir à court terme des enfants déclarés sortants après consultation aux urgences médicales pédiatriques. Méthode : Etude prospective réalisée en 2003, dans un service d'urgences pédiatriques médicales. Inclusion de 301 enfants déclarés sortants. Enquête téléphonique 48 h après la consultation. Résultats : Un cas a été perdu de vue. L'âge moyen était de 3,4 3,3 années. 48 h après consultation dans ce service d'urgences médicales pédiatriques : 82% des enfants bénéficiaient d'une prescription du service ; Le taux d'adhérence au traitement était de 95% ; Le taux de re-consultations était de 22% ; 82% des re-consultations ont conduit à modifier la prise en charge ; Le taux d'hospitalisation était de 2% ; Il n'y a pas eu de décès ni d'hospitalisation en réanimation. Conclusion : Les prescriptions sont très bien suivies, les re-consultations paraissent justifiées et les décisions de non-hospitalisation semblent pertinentes.
