The intent of this study was to cross-validate a previously evaluated assessment tool designed for the purpose of detecting genuine from feigned PTSD as well as to provide some of the first data exploring the effects of coping style, resilience, and social support on one's ability to feign PTSD. PTSD is a highly compensable disorder of which diagnosis is largely dependent on the subjective report of associated symptoms, making it particularly susceptible to malingering. Malingering, often viewed by mental health professionals as an antisocial act, can have severe consequences for the individual who is detected as feigning. More recent theories, however, have noted the potential utility of malingering under specific circumstances and have suggested that malingering may be an extreme form of coping. When viewed in this light, malingering can be seen as an adaptive quality rather than antisocial behavior, which can impact not only the clinical view of malingerers, but also the treatment options for these individuals. The current study sought to provide one of the first evaluations of the impact of one's coping response style on one's ability to feign PTSD. Also, because of the positive relationship between coping, resilience, and social support, the current study also sought to provide one of the first evaluations of the impact of each construct (i.e. coping, resilience, social support) on one's ability to malinger. The current study also explored the complex relationship of all three constructs on one's ability to feign PTSD and whether or not specific combinations allow for optimal malingering ability. This study employed a simulation design with a sample of college undergraduates with and without genuine PTSD. Coping response style, degree of resilience, and social support were also evaluated for all participants. Participants' ability to feign, as calculated by the experimental measure, was compared across coping, resilience, and social support. The ultimate goal of this study was to explore these relationships and provide empirical support for understanding malingering as an adaptive coping mechanism.

It presents a new model of malingering and related biases, and develops a “diagnostic” system based on it that is applicable to PTSD, chronic pain, and TBI. In Malingering, Feigning, and Response Style Assessment in Psychiatric/Psychological Injury, Dr. Young ambitiously sets out to articulate and synthesize the polarities involved in the assessment of response styles in psychological disabilities, including PTSD, pain, and TBI. He does so thoroughly and very even-handedly, neither minimizing the degree that outright faking can be found in substantial numbers of examinees, nor disregarding the possibility that there can be causes for validity test failure other than malingering. He reviews the prior systems for classifying evidence of malingering, and proposes his own criteria for feigned PTSD. These are conservative and well-grounded in the prior literature. The feigning of psychiatric symptoms is of great concern in both clinical and forensic settings. Therefore, it is crucial to develop reliable and valid measures that are not only diagnostically valid but also allow for the detection of individuals who are attempting to exaggerate illness in order to receive monetary compensation or escape duty or work. The present study was initiated so as to assess the psychometric properties of a new measure for the assessment of Post-Traumatic Stress Disorder (PTSD), the A-PTSD Scale. This 35-item self-report measure relies on the DSM-V criteria and employs indirect questioning as well as reversed items to provide more security against the feigning of symptoms. The A-PTSD scale utilizes two subscales: the primary PTSD scale which provides a total score, and a resiliency scale. The resiliency scale is intended to work as both a prognostic measure and a validity scale. Namely, there is ample research showing that individuals with such traits can successfully recover from adverse experiences. As such, they are less likely to suffer from long-term chronic PTSD symptoms. Given that stand-alone PTSD measures are impacted by confirmatory bias the A-PTSD scale was intended to be embedded within the 148-item CAP-Q, a multiscale self-report measure that includes its own traditional validity scales. The results of the study show that A-PTSD psychometric properties are not impacted by being embedded within the CAP-Q. It also had comparable reliability with existing PTSD measures and was able to discriminate between PTSD simulators and non-simulators, correlated with cognitive measures of effort. Overall, the A-PTSD appears to have incremental utility over traditional measures, although additional studies using clinical populations are recommended prior to adapting measure into clinical practice.

The intent of this study is to cross-validate an assessment tool designed to identify genuine and feigned PTSD as well as to provide some of the first data available on the ability to feign PTSD among those with past experience with the associated symptoms. PTSD is one of the most highly compensable mental health disorders and its diagnostic criteria are readily accessible to the public, which makes the disorder highly vulnerable to feigning. Current assessments designed to identify PTSD often lack the ability to differentiate feigned from genuine PTSD. The current measure was designed to fill this gap. Recently, the effect of coaching on the ability to feign PTSD has been a focus of research, essentially studying the effect of knowledge of PTSD on the ability to feign PTSD. This research has largely ignored one critical form of knowledge: personal experience of PTSD or the associated symptoms. The current study seeks to examine the effect of personal experience of PTSD on the ability to feign PTSD. This study employs a simulation design with a sample of college undergraduates with and without genuine PTSD. Participants asked to feign PTSD, who do and do not have lifetime PTSD experience, are compared to honest responders with PTSD. Preliminary results indicate that individuals with experience of subthreshold or remitted PTSD were as easily detected by the measure as individuals with no lifetime PTSD experience.
